**THE OPPORTUNITY**

We are looking for a highly motivated and organized lifestyle health coach to provide telephone coaching as part of an overall wellness and population management program. Lifestyle health coaches provide an individualized health improvement program to participants in brief sessions, typically 15 minutes, over the telephone. Coaches are trained to deliver a standardized, evidence-based program that addresses nutrition, weight management, physical activity and exercise, stress management, tobacco cessation, risk factor reduction (blood pressure, cholesterol, triglycerides), diabetes self-management, sleep/fatigue management and preventive exams and immunizations. The lifestyle management program is supported by a computerized tracking and management system and comprehensive educational materials.

**THE SKILL SET**:

- Bilingual - English/French
- Bachelor's degree in a health-related field
- Excellent time management skills; the ability to manage many diverse workflows and implementations at once
- Two years of relevant experience in a health-related field
- Intuitive mindset with the proven ability to creatively problem-solve in ambiguous environments
- A caring and compassionate attitude
