The ADT Coordinator is accountable to the Patient Flow Manager for the coordination of timely and safe admission of patients and clients to Providence Hospital. The ADT Coordinator provides seamless, prompt, friendly customer-focused service to incoming patients during the admission & discharge processes, as well as, handling inquiries from patients and the general public at Providence Healthcare. All duties are performed in accordance with the Mission and Values at Providence Healthcare.

**MAJOR RESPONSIBILITIES**:

- Answer telephone, screen calls, respond to routine inquiries and take messages in accordance with the clinical team instructions;
- Acts as first line of inquiry to patients and visitors and projects a welcoming, congenial and supportive attitude befitting the mission and values of Providence Healthcare;
- Oversees and performs a range of administrative duties in coordination with PFCs, clinical staff and PCMs;
- Communicates admissions/discharge/transfer information between the Patient Flow Coordinators (PFC), Patient Care Managers (PCM), Pharmacy and Unit Administrative Coordinator (UAC) to ensure maximum utilization of program beds;
- Manages internal and external waitlists (i.e. RMR, E-STROKE);
- Coordinates process of gathering and compiling patient personal health and confidential information upon admission;
- Coordinates the logistics in transition from the referring source to admission to our programs;
- Ensures the consistent and timely completion and processing of Admission Agreements, including Preferred Accommodation payment information;
- Ensures that once the Patient Flow Coordinator (PFC) has reviewed the referral, that any follow up telephone calls, faxing or data entry that is required, is completed in a timely fashion;
- Tracks and obtains missing/additional information necessary for effective planning of patient care;
- Gathers inpatients’ primary care provider information attached to their personal health records in Meditech data base; as well as helping to facilitate connections to primary care for those patients who are not attached to a primary care provider;
- Enters all Community Health Navigators appointments into Community Wide Scheduling;
- Set up and Manage bi-monthly Wheeltrans clinic with patient appointments;
- Maintains accurate record of department attendance for purposes of payroll and attendance monitoring;
- Works collaboratively with finance/billing team to optimize the hospital Preferred Accommodation revenue generation;
- Support the Health Equity Data Collection process as per the Toronto Central LHIN for Providence Hospital;
- Maintains effective and efficient filing record keeping systems.

**QUALIFICATIONS/SKILLS REQUIRED**:

- Successful completion of a related post-secondary/community college program or comparable skill level;
- Five years’ experience in a health care environment, preferably in a similar role;
- Knowledge of PHIPA;
- Knowledge of medical terminology;
- Detail oriented with excellent organizational skills;
- Excellent interpersonal and communication skills; verbal and written;
- Experience with Meditech software is a definite asset;
- Demonstrated Client Service focus/approach;
- Knowledge and ability of community resources to ensure the correct information is given when a client is seeking information from Providence Healthcare over the phone or at the door;
- Ability to work in collaboration with the inter-professional team and various departments in the facility;
- Ability to community effectively and exercise good judgment in all circumstances related to the public, patients, clients, family members/SDMs while remaining calm and professional when under pressure;
- Able to work independently, balanced with proven track record as a team player;
- Demonstrated conflict resolution skills;
- Very good attendance record;
- Good performance record.
