Reporting to: Director of Community & Supportive Care

**POSTION SUMMARY**
St. Joseph’s Hospice is a charitable, faith-based organization that offers compassionate holistic care and
support to people living with a palliative illness. Support is extended to their family members and
caregivers and to those grieving the loss of a loved one (Client*). Hospice programs and services are
offered at no cost and are available irrespective of age, gender, religion, race, ethnicity or economic
status.
Whether in the community, our wellness centre or our 10-suite residence, St. Joseph’s Hospice provides
specialized programs and services with a person-centred holistic approach. Individuals and families are
intimately involved in creating their own plan of care which best addresses their unique needs. Our
competent and committed Hospice staff and volunteers work together as a team, with other community
partners and service providers, to provide full support and quality hospice palliative care.
Working within an interdisciplinary team, and in accordance with the Mission, Vision and Values of St.
Joseph’s Hospice, the Program Coordinator, Community & Supportive Care is responsible for
ensuring that Hospice Clients and family members receive exemplary supportive care. This approach will
meet the needs of everyone Hospice serves, including persons of faith and those who have no religious
affiliation or practice.
Additionally, this individual ensures that all health and safety standards are met and continually reinforced.
This individual will work in a safe manner and will promote the safety of all co-workers, volunteers, clients,
residents, families and visitors.

**MAJOR RESPONSIBILITIES**

**PROGRAM COORDINATION, DELIVERY and EVALUATION**
Meets the diverse needs of those we serve, providing leadership for support groups and wellness
programs under St. Joseph’s Hospice Supportive Care programming.
Responsible for the quality and facilitation of support groups and wellness programs.
Responsible for training of volunteers and the scheduling of volunteers and staff to deliver
evidence-based, values-informed and person-centered support groups and wellness programs.
Responsible for the recruitment, assessment, placement and monitoring of all participants
interested in groups and activities, making appropriate internal and/or external referrals

**ASSESSMENTS and CARE PLANNING**
Functioning as part of the interdisciplinary team ensures that Hospice Clients are assessed for services
and programs under the Community & Supportive Care umbrella and are aligned with the Domains of
Issues Associated with Illness and Bereavement. Additionally the Program Coordinator:
Incorporates the use of validated tools as part of Client intake/assessments; offering a
compassionate presence, access to Hospice programs and services and services.
Utilizes assessments to provide the framework for interdisciplinary team communications,
decision
- making, care planning and the delivery of an integrated overall Client care plan;
Communicates readiness of individuals groups and other services
Conducts Supportive Care intakes
Works in collaboration with the inter-disciplinary team to provide supportive care to Hospice
Clients,
Supports and supervises volunteers as appropriate
Maintains accurate and confidential records, in accordance St. Joseph’s Hospice policies and
procedures.

**EVENTS**
Coordinates community dinners, bereavement luncheons, and memorial services.

COMMUNITY
Upholds the Catholic Mission weaving it into the fabric of St. Joseph’s Hospice by fostering
hospitality, celebration, compassionate care, and respect for religious, spiritual and cultural
diversity throughout the organization.
Represents St. Joseph’s Hospice in a positive and professional manner and promotes the best
interest of the organization through speaking engagements and participation on external
committees, task forces, networks, or other meetings as required.

**PROFESSIONAL RESPONSIBILITY**
Adheres to policies, procedures and best practice standards of St. Joseph’s Hospice.
Understands and promotes the culture of philanthropy at Hospice and serves as a philanthropic
ambassador within the organization and in the community.
Performs all other duties as assigned from time to time.

**QUALIFICATIONS**:
To perform this role successfully, an individual must be able to perform essential duties satisfactorily. The
requirements listed below are representative of the knowledge, skills, and /or abilities required.
University degree or College diploma in Social Work or related field.
Certificate in Grief and Bereavement or Thanatology from a recognized post-secondary institution
preferred
Experience in a not-for-profit setting and demonstrated organizational skills an asset.
