**Highlights**:
As an Inclusion Facilitator, you will be working closely with the Manager and Team Leader, Individual(s) supported and their networks to collaborate and support the development, implementation, and evaluation of person-centred planning and services. This role is focussed on supporting individuals to gain or grow toward finding paid employment, making new connections and friendships, acquiring new skills, and developing increased independence, within the community of their choosing.
**Duties and responsibilities**:
- You will be responsible for supporting the person to build skills in areas such as employment search/maintenance, informal support network strengthening, friendship building, community connection, skill and knowledge development.
- You will have the ability to facilitate job search activities to assist individuals in designing self-marketing tools including resume development, cover letters, references, and interviewing techniques, etc.
- connect individuals’ to employers, community activities, and learning opportunities that match the interests, skills, and abilities of individuals.
- Provide on-the-job support in order to ensure sustained success whenever necessary.
- Connect the person with adaptive technology and assist employers with workplace accommodations, as needed. Understand available subsidies to help a person get work.
- Develop and implements support strategies within program guidelines in consultation with the Manager and Team Leader.
- Maintains records and statistics for each person supported and provides reports to the manager and team leader as required.
- Work with all stakeholders to promote the interests of people supported with other community service providers, professionals, school/college/university personnel, and employers as required.
- Provide transportation/accompany individuals’ to meetings and appointments as required.
- Gather information relevant to the person’s goals and life expectations through interviewing the person, observing their behaviour, meeting with families, caregivers, and service providers.
- Review and assess the information gathered to develop plans and strategies to achieve goals and pursue life expectations, as well as mitigate risks, develop solutions to barriers, and meet needs.

**Driving Requirement**
Valid BCDL and vehicle will be considered an asset, not required.
**Preferred Training or Experience**
One year of recent related experience or an equivalent combination of education, training, and experience is required.
Preference will be given to applicants with education in a related discipline such as Human/Social Services, Community Support and/ or Rehabilitation Services or equivalent combination of experience.

35 hours per week (must be flexible).

**Salary**: $26.20-$29.90 per hour
