**Service Coordinator (Permanent Full Time)**

**About Us**

Traverse Independence primarily supports people with acquired brain injuries (ABI) to live independently in the community. As a centre of excellence, we empower and connect clients on their journey to independence. We offer services across the continuum of care, including those to people who are experiencing homelessness or are living rough, mainstream outreach in people's homes, transitional living, supportive housing, and drop-in centres. Our programs are low barrier and focus on the social determinants of health, practical issues, and concerns with an intent to keep people stabilized, housed, and living in the community.

**Job Overview**

Reporting to the Program Manager, the Service Coordinator will be responsible for:

- Directly supervising front line staff by providing coaching, team building, supervision, performance reviews and performance management
- Oversight and evaluation through data collection, of both individual client service and the overall program, providing leadership to ensure equitable access to high quality services
- Notifying the manager of client and staff issues or concerns that cannot be resolved immediately, or that are considered a high priority item
- Support all the procedures and processes required to maintain an exemplary standing with Accreditation Canada
- Value and capably use a fully integrated SharePoint, O365 environment
- Support the health and safety of both staff and clients
- Ensure clients who are in a goal-based program have all the required assessments, documentation and service plans completed
- Ensure integration with other sectors, and workers involved in client care team to support wrap around services for clients
- Maintain a professional, collaborative, and integrative perspective and culture in program
- Work within an ethical framework, risk management system, and quality improvement philosophy
- Have a clear and dedicated understanding of privacy and confidentiality.

**Requirements**:

- Community College Diploma in Social Services, Healthcare, or other related discipline
- A minimum of 2 years' experience in social services or healthcare with at least 1 year supervising or leading a team
- Experience in the mental health and/or addiction
- Demonstrated experience in behaviour management programs
- Comprehensive knowledge and experience working in a SharePoint environment, including Microsoft 365 Teams (GoldCare experience a benefit, but not necessary)
- Problem solving and conflict resolution skills
- Excellent communication skills (written and oral)
- Experience working with persons with disabilities and/or acquired brain injuries
- Understanding of community resources, with an emphasis on mental health and addictions, along with issues that relate to substance use, mental health, and brain injury
- Familiarity with health and safety requirements, AODA and WHMIS
- Must have valid Class G driver's license and reliable vehicle. Travel will be required.
