The Patient Flow Manager engages clinical leadership and support services in order to achieve Providence Care Hospital (PCH) and Providence Transitional Care Centre (PTCC) clinical objectives and maximize operational effectiveness. The Patient Flow Manager is functionally responsible for ensuring that the structure and processes are in place to support safe, effective, and efficient admissions, transfers and discharges at PCH/PTCC. Through collaboration with key patient care service partners, both internal and external, the Manager supports the goal of patients/clients receiving the right care at the right time and for the appropriate duration. This includes utilization of related health care resources, conducting/reviewing data analysis and preparing management reports.

**Education & Experience**:

- Master’s Degree in a Health Care Discipline or an equivalent combination of relevant education and experience may be considered; nursing and/or social work discipline education is an asset
- Five years of experience in health care management, including administration of policies and procedures, leadership of staff members from social work and/or nursing domains, and working closely with physicians
- Knowledge of health care utilization demands and related financial forecasting, analysis and statistical reporting experience is an asset
- Knowledge of LUMEO product, planning and implementation an asset

**Position Duties & Responsibilities**:

- Provides leadership and direction to Access to Care and Transition Specialists, Social Workers, Social Service Workers, Patient Flow Coordinators, and works closely and collaboratively with other staff affiliated with access to care and transitions
- Works with internal stakeholders (ie management, staff and physicians) to improve the safety, efficiency and effectiveness of patient transition processes including those related to referrals, intake, admissions, discharge and transfers in all programs excluding Forensic Mental Health and Palliative Medicine
- Ensures alignment of Alternate Level of Care (ALC) designation practices and processes to system level initiatives, relevant legislation and stakeholder expectations
- Fosters program growth and development through excellent advocacy skills and the ability to generate innovative solutions both within the hospital and the broader community
- Collaborates to manage the development and implementation of programs that foster links with the community to maximize the continuity of care between Providence Care and the community
- In collaboration with relevant stakeholders, identifies, monitors and sets key performance targets related to access to care and transitions (e.g., occupancy, Length of Stay (LOS), patient days, ALC, predictive discharge targets, referral and admission wait times)
- Leads the recruitment of new staff for the interprofessional Patient Flow team, working with support from People Services to coordinate recruitment, interviewing, hiring and orientation
- Manages performance of direct reports including, ongoing coaching, mentoring, counseling and/or disciplinary action and performance management, involving other relevant stakeholders as appropriate (e.g. People Services, Director(s), Professional Practice, Quality & Risk Management)
