**DESCRIPTION**

The position supports KHSC quality measurement and reporting, safety and quality improvement, appropriate tracking of patient feedback, and effective management of safety incidents through oversight of patient safety incident reporting, extracting and analyzing information, and administration of the KHSC Incident Management System (herein referred to as SAFE).

The Patient Safety and Quality (PSQ) Analyst works closely with members of the Patient Safety Quality and Risk (PSQR) team to implement improvements to SAFE reporting in response to changes in KHSC clinical services, emerging and evolving risks to patient safety, and changing information needs. The PSQ Analyst also adapts system configuration to leverage software enhancements, improve functionality for KHSC users, and streamline incident tracking and patient feedback documentation

The PSQ Analyst contributes to the implementation of an integrated quality reporting plan to support quality and risk monitoring and management at all levels of the organization; assesses and addresses data quality issues, identifies and assesses new and emerging trends in patient safety reports; develops reports and provides analysis of patient safety and patient experience related data.

The position requires a general understanding of quality, patient safety, and risk management practices in a healthcare setting and just culture, as well as data literacy and competency with navigating and troubleshooting software systems.

**PRIMARY RESPONSIBILITES & DUTIES**:

- System Administrator for the KHSC Incident Management System (referred to as SAFE) ; ensuring software is configured to meet organizational requirements and ensure the ongoing integrity and functionality of the system.
- Maintain and adapt software configuration in response to operational changes (e.g. user profiles, forms, alerts, auto-reports, workflow).
- Troubleshoot configuration and end user issues, and engage appropriate business and technical resources to coordinate resolution.
- Analyze system change requests to ensure ongoing integrity of the system and continued integration and compatibility with unique PSQR and Occupational Health components of the database.
- Collaborate with Information Management department to coordinate vendor software upgrades and minimize disruption to system users
- Ensure data and system access requests are managed within appropriate privacy and confidentiality policies.
- Ensure data quality for reported safety incidents
- Maintain incident reporting framework documentation
- Perform data quality audits to ensure accuracy, inform user education, and implement corrective action
- Recommend and implement improvements to SAFE reporting forms and workflows in response to changes in policy and process requirements, reporting requirements, and feedback from system users.
- Contribute to the development and implementation of an organizational reporting plan that integrates quality and risk monitoring and management at all levels of the organization.
- Provide education and training to SAFE users at all levels at KHSC regarding functionality and use of incident reporting software, and interpretation of the reporting framework.
- Assess education needs and develop education materials to meet the identified needs.
- Develop and deliver corporate and clinical/program specific orientation on how to use incident reporting software to report incidents and near misses; provide additional training to new leaders on file management and closure.
- Adapt and deliver education and communication in response to changes to patient safety incident reporting
- Act as corporate resource to provide ad-hoc support to ensure efficient and effective patient safety incident reporting
- Contribute to advancement of effective patient safety, quality and risk management at KHSC.
- Contribute to the ongoing development and implementation of quality and risk management frameworks and supports at KHSC (e.g. policies, definitions, processes, tools, education etc.)
- Promote a ‘just culture’ of safety at KHSC that encourages personal responsibility, open and transparent communication about safety, and systemic improvement

**QUALIFICATIONS**:

- 3 year diploma in a health or information management discipline; equivalent combination of education and experience may be accepted
- Bachelor’s degree with appropriate prerequisite knowledge, experience and skills preferred.
