The Clinical Navigator is a member of the Patient Care Team who actively strives to serve as a patient advocate, educator and support person for patients diagnosed with CHF and COPD. They collaborate with an interdisciplinary team that includes staff from LHSC and our community partners (e.g. Primary Care, St. Joseph’s Health Care-London, Home and Community Care) to facilitate a smooth and seamless transition for patients with CHF and COPD across the health care continuum. The Navigator uses the best evidence available to support patient and/or family education to promote self-management for optimal health for patients diagnosed with CHF and COPD.

Regular Full Time

Rate of Pay: $44.33 per hour to $55.41 per hour

Hours of Work: 37.5 hours per week

**Qualifications**:

- Successful completion of an Undergraduate Bachelor's Degree in a health-related field
- Current certificate of registration from a college recognized under the Regulated Health Professionals Act
- Proof of certificate of completion of a COPD Educator Program preferred
- Project Management Certification preferred
- Minimum five (5) years of recent, related experience
- Demonstrated advanced clinical expertise in COPDor CHF Care and managing COPD or CHF patients
- Demonstrated advanced planning and evaluation skills
- Advanced skills in project and change management
- Demonstrated advanced facilitation and consensus building skills
- Proficient with computer skills including word processing, power point presentations, Microsoft project, spread sheets, and graphics
- Advanced problem solving and decision-making skills
- Demonstrated ability to balance multiple and competing priorities on a daily basis
- Demonstrated knowledge of local and regional health care systems
- Advanced interpersonal and communication skills, both written and verbal
- Demonstrated practice and commitment to the principles of patient and family centered care
- Demonstrated practice and commitment to patient and staff safety at LHSC
- Demonstrated practice and commitment to LHSC’s Vision, Purpose and Values
- Demonstrated ability to attend work on a regular basis

Immunization Requirements:
Before beginning employment at LHSC, all new hires will be required to:

- Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox), Hepatitis B, Tetanus/ Diphtheria/Polio; Meningitis;
- Provide documentation of the Tuberculosis skin testing (two step);
- Provide proof of Health Canada-approved COVID-19 vaccination reflective of Ontario Health’s current definition of being fully vaccinated
