**Clinical Navigator - Children's Care - Haemoglobinopathy Program**

The Clinical Navigator serves as a patient educator, advocate, and support person for patients who may benefit from health care navigation. The Clinical Navigator collaborates with an interdisciplinary team that includes staff from LHSC and our community partners to facilitate a smooth and seamless transition of patients across the health care continuum. This role uses the best evidence available to support patient and/or family education to promote self-management for optimal health for patients.

The London Health Sciences Centre (LHSC) Haemoglobinopathy Program is a regional program that provides comprehensive care for children and adults with sickle cell disease and thalassemia and support for their families. There has been significant growth in the number of people with haemoglobinopathies living in Southwestern Ontario. This newly established program will provide multi-disciplinary, lifelong care to patients with haemoglobinopathies, serving a catchment area of 2.6 million people.

The Clinical Navigator
- Transitional Care, will assist children and adolescents with hemoglobinopathies to effectively transition from childhood to adulthood and from the child to the adult health care system. You will provide and promote excellence in transition patient care, patient and staff education, consultation, and research. The Transition Specialist will work with professionals from both pediatric and adult hemoglobinopathy programs. You will play an important role in representing and promoting the program’s goals, objectives, activities and research initiatives.

Rate of Pay: $40.27 per hour - $50.34 per hour

Regular Full-Time

**Qualifications**:

- Undergraduate Bachelor’s Degree in a health related field or equivalent acceptable to the hospital
- Current certificate of registration from a college recognized under the Regulated Health Professions Act
- Minimum of 5 years recent related experience in the care of paediatric and/or adult patients with haematology disease
- Proficient Microsoft Office skills
- Advanced knowledge of local and regional health care system
- Advanced critical thinking and decision-making skills
- Advanced people management skills
- Ability to build collaborative partnerships
- Demonstrated strength in interpersonal relations and communication skills, both verbal and writte
- Demonstrated proficiency in planning and evaluation
- Demonstrated facilitation skills e.g. consensus building
- Demonstrated computer proficiency with clinical systems
- Demonstrated practice and commitment to the principles of patient and family centred care
- Demonstrated practice and commitment to patient and staff safety
- Demonstrated practice and commitment to LHSC’s mission, vision and values
- Demonstrated ability to attend work on a regular basis
