This role provides an exciting opportunity to support those who have acquired brain injury (ABI), their families, caregivers, and service providers in navigating ABI services throughout Southwestern Ontario (SWO). This position aims to improve the access and coordination of services for clients with ABI and is an ideal opportunity for an individual who enjoys being on the leading edge of innovation with a view of improving the ABI services delivery system. The Navigator will work closely with ABI services across the continuum of care (acute, rehabilitation, and community) and cross-sectoral services (e.g. housing, mental health and addictions, justice, etc.) that support individuals with ABI. The role will combine clinical skills with service coordination, collaboration and partnership creation skills with stakeholders, resources and services across SWO, and involves some travel throughout SWO.

The key roles for this position will include:
Develop working relationships and partnerships with services and agencies (both ABI and appropriate non-ABI) in order to assist with the coordination of ABI service needs
Provide recommendations to clients, families, and care providers about services available across the continuum for clients with ABI and multiple, complex needs
Support education, capacity building, and system/quality improvement across all sectors that support individuals with ABI
Assist with the transition of ABI clients across the continuum (typical clients include a diagnosis of ABI as well as concurrent mental health and/or addiction issues)

Additionally, the incumbent has responsibility for and Co-Chairs the ABI Network for Southwestern Ontario, a group of cross-sectoral providers across the continuum and across the ten counties of South Western Ontario.

**Essential Qualifications**
- Successful completion of a Bachelor's Degree in Nursing or Allied Health Profession
- Valid registration with a regulated health profession college
- Minimum of 3 years clinical experience in the field of ABI rehabilitation
- Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
- Ability and commitment to act as a supervisor under OHSA
- Demonstrated knowledge of ABI and its consequences, rehabilitation process and philosophy
- Demonstrated community development skills
- Travel within the catchment area is required
- Excellent skills in consultation; facilitation; negotiation; mediation and client advocacy
- Advanced interviewing skills
- Creativity and initiative to complement strong problem solving skills
- Superior verbal and written communication skills; interpersonal and leadership skills
- Ability to work collaboratively with many internal and external individuals, groups and agencies
- Excellent conceptual, analytical and evaluative skills
- Preferred Qualification
- Proficiency in French would be an asset
- Provide documentation you have received two doses of the Covid-19 vaccine (primary series, boosters and/or XBB) OR one dose of XBB vaccine at least 14 days prior to the start date.
- Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox)
- Provide documentation of the Tuberculosis skin testing
