The Regional Navigator, Specialized Geriatric Services will support day-to-day operations of regional clinical teams, development and stabilization of processes to expand and ensure equitable access to specialized services across the South West region, and facilitate collaboration between partners regarding system navigation

Duties will include:
Developing and reinforcing collaborative processes for the provision of best practice, integrated care for older adults across the region
Identifying emerging issues impacting the delivery of integrated, high-quality care across the region; addressing and/or elevating issues for leader follow-up, as appropriate
Identifying opportunities for improvement/change in clinical practice and team processes
Facilitating and leading collaborative conversations and make decisions related to the navigation, triage, and prioritization of patient referrals
Developing and maintain positive relationships with relevant stakeholders, including referral sources, specialized service providers, and community partners
Supporting team functioning and service delivery in alignment with regional and provincial strategic directions, priorities, and best practices
Serving as a first point of contact to support staff in their day-day clinical care, around issues such as:
Scheduling & referral triage and prioritization
Collaboration with partner organizations/providers
Clinical best practice
Safety protocols & space/equipment needs
Program and/or Corporate policies

**Essential Qualifications**
- Baccalaureate Degree in a health-related discipline
- Current registration with applicable College
- Broad perspective and an ability to think strategically as well as operationally; and proven ability to lead quality improvement projects, with a high competency in teaching and coaching others.
- Well-developed leadership, interpersonal, communication, conflict resolution and organizational skills
- Proven flexibility and comfort working in ambiguous environments; ability to work independently and take initiative, with mínimal supervision. Proven organizational skills with the ability manage multiple projects and constantly adjust priorities.
- Ability to manage competing demands combined with the ability to work both collaboratively and independently.
- Knowledge transfer and translation skills, and experience with synthesizing information and leading the adoption of evidenced based practice into practice settings; and evaluating the success of those strategies
- Ability to oversee and build working relationships with teams and partners that are located across a wide geographic region, and ability to engage system partners at varying levels, including managers, physicians, and front-line staff.
- Competent and effective presentation and meeting facilitation skills including the ability to present and discuss complex information in a way that enables and establishes rapport and persuades and influences others.
- Demonstrated understanding of the health-care system and issues and awareness of unique challenges experienced between and within the various services: hospitals, primary care, and community and outreach services.
- Demonstrated excellence in written (including report writing) and verbal communication skills, including the ability to demonstrate tact and diplomacy with varied groups of stakeholders.
- Competent in the use of computer software, particularly word-processing and presentations and comfortable working in a virtual environment
- Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA

**Immunization Requirements**
- Provide documentation you have received two doses of the Covid-19 vaccine and a written commitment to follow any future Public Health recommendations and St. Joseph's Health Care London policies and processes related to Covid management in the workplace.
- Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox)
- Provide documentation of the Tuberculosis skin testing
