Oncology Patient Navigation Program (OPNP)

The Oncology Patient Navigator works as part of the Oncology Patient Navigation Program, which aims to improve the quality and accessibility of the diagnostic phase for all individuals undergoing a potential cancer diagnosis. Utilizing a person-centred approach, the Oncology Patient Navigator will be responsible for managing and coordinating care that spans from referral due to cancer suspicion to diagnosis. This will include supporting diagnostic assessment according to evidence-based clinical pathway maps, streamlined scheduling and coordination of diagnostic tests and consultations, relevant and timely patient education throughout the diagnostic process and improved care transition and communication between healthcare providers.

Regular Part-Time

Rate of Pay: $43.04 per hour - $53.80 per hour

Hours of Work: 37.5 hours per week

**Qualifications**:

- Successful completion of a Bachelors Degree in Health/Social Science, Quality Improvement, Change/Risk Management, Health Management, or equivalent experience as accepted by Hospital
- Current certificate of registration from a college under the Regulated Health Professions Act
- Patient Navigation Course and/or Lean certification (Green or Black Belt) preferred
- Minimum 5 years recent clinical experience in Thoracic surgery, Respirology or Oncology
- Demonstrated knowledge of local and regional health care systems
- Demonstrated excellent planning and evaluation skills
- Proficiency with computing skills including the use of such programs as Microsoft Office (Word, Excel), PowerPoint Presentations, Cerner
- Demonstrated excellent facilitation skills, i.e) building of collaborative partnerships, project and change management, problem solving and decision making skills
- Demonstrated excellent interpersonal and communication skills, both verbal and written
- Effective organizational skills involving time management and prioritization
- Proficient in leadership and vision with the ability to empower and enable others
- Demonstrated people management skills
- Proven flexibility, adaptability and ability to manage workload and change in a dynamic environment
- Proficient in building collaborative partnerships
- Demonstrated practice and commitment to the principles of patient and family centered care
- Demonstrated practice and commitment to patient and staff safety at LHSC
- Demonstrated practice and commitment to LHSC’s Mission, Vision and Values
- Demonstrated ability to attend work on a regular basis

**Immunization Requirements**:

- Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox), Hepatitis B, Tetanus/ Diphtheria/Polio; Meningitis.
- Provide documentation of the Tuberculosis skin testing (two step) and COVID vaccine series.
