Currently recruiting a Clinical Navigator to support a new diagnostic software platform known as Actic. The software platform focuses on eliminating musculoskeletal disability with rapid precision diagnostics and treatment. Actic sets patients on the right track to health through a holistic assessment and identification of barriers to recovery.

The Clinical Navigator will connect with our clients to ensure they receive appropriate information, treatment, and support along their path to better health. We are passionate about improving our clients lives and employing caring and skills providers to help our clients navigate the path to wellness.

**Job Description and Primary Responsibilities**
- Complete intake assessments for all clients referred
- Work with clients to create a treatment plan
- Assist clients in identifying, navigating, and connecting with the appropriate rehabilitation resources
- Point of Contact for the clients Case Managers
- Other duties as assigned as might be required as a result of organizational development

**Qualifications**:

- Occupational Therapist currently registered with the College of Occupational Therapists of Ontario
- Demonstrated ability to provide comprehensive assessments to clients
- Ability to problem solve and demonstrate critical thinking skills
- Strong understanding of rehabilitation resources and supports available
- Exceptional communication skills
- Demonstrated ability to problem-solve with good judgement
- Some flexibility with scheduling
- Strong organizational skills

**Salary**: From $70,000.00 per year
