JORDAN’S PRINCIPLE FAMILY SERVICE COORDINATOR

**Job Description**:
The Prince Albert Grand Council is a non-profit organization that provides a range of programs ands services to 12 First Nations communities.

Under the Supervision of the Health Director, or designate, the Jordan’s Principle Service Coordinator will be part of a multi-disciplinary team of health professionals and support staff.

QUALIFICATIONS: The Jordan’s Principle Service Coordinator will work collaboratively with families by:

- building relationships with service providers, and federal or provincial programs and services
- ensuring children receive quality and culturally appropriate health and social services
- networking and making connections for families with children with special needs
- ability to advocate for proper support services for families and children
- submitting completed requests to the Jordan’s Principle Regional Focal Points for review
- managing funds provided through Jordan’s Principle for services/supports approved for families
- supporting data collection and analytical activities to better understand the scope of children’s needs and nature of service gaps
- Recognized health or social degree, in good standing with respective regulatory body
- Knowledge of Federal/Provincial Health Care Systems.
- Knowledge of Jordan’s Principle - Child first initiative
- Knowledgeable and respect for First Nations cultural practices, traditions, and customs
- Must have excellent oral (including public speaking) and written skills
- Must be computer literate and have experience working with computer programs
- CPR and First Aid certification
- Must have a valid Saskatchewan Drivers License and be willing to travel
- Must provide a Criminal Record Check and Vulnerable Sector check
