_Transitions is a non-profit charitable organization supporting children with developmental delay and/or disabilities.
The Children and Family Services Program at Transitions believes that children thrive in a nurturing family and supportive community. Our goal is to strengthen the family’s ability to raise their child by providing a variety of supports._
- **The Family Services Worker (FSW) is responsible for coordinating various supports for families, mentoring Child and Youth Support Workers (CYSW), and assisting families to connect with their community based on the interests and gifts of their child/youth.**_
- **The Part Time Family Service Worker position is 14.5 hours per week, 2 shifts working 8:30am - 4:30pm, Monday
- Friday.**_
Duties and Responsibilities:
- Coordination of supports to assigned families: case management.
- Collaboration with families to review their staffing/support needs, renew consents, update required information, and maintain regular contact and communications.
- Coordinate the matching of staff and families based on skill and compatibility.
- Accompany newly hired Child and Youth Support Workers to the family’s home on their initial introductory shift and additional shifts if required.
- To be available to mentor CYSW's in a family’s home or in the community as needed.
- Collaboration with the Family Service team and the Supervisor of Family Supports with the coordination of Summer Day Camps that are community based and fully inclusive.
- Implementation and coordination of after school youth supports as needed.
- Collaboration with community partners to increase opportunities for children and youth to be active participants within their communities.
- To be knowledgeable about the services and activities available to children and youth within St. Albert and Sturgeon County. To develop and maintain an inventory of these resources and make this available to all staff and families.
- Start up and development of resource program for parents as their child reaches the age of 16, in preparation for adult supports at 18 years of age.
- To establish and maintain case files for each child, ensuring all files are kept confidential, organized and current. To maintain a well-organized work site.
- To prepare and submit required statistical information monthly.
- Participation as a contributing team member.
- To assist in monitoring program effectiveness and parent satisfaction.
- Assume other duties and responsibilities as required.
Qualifications/Knowledge/Skills/Abilities/Competencies:
- Diploma/Degree in Community Rehabilitation or a related field of study.
- Experience working directly with children and families, preferably children with disabilities/developmental delays.
- Excellent interpersonal and communication skills.
- Ability to work effectively both independently and within a team.
- Creative problem solving skills.
- Motivated and reliant.
- Exceptional organizational skills.
- A satisfactory Criminal Record Check and Child Intervention Record Check.
- Valid Drivers License and a reliable vehicle.
**Job Types**: Part-time, Casual, Permanent
Part-time hours: 14.5 per week
**Salary**: $22.55-$27.44 per hour
