**Role Overview**
- The Manager, Community Health Services is accountable for providing leadership in the development and delivery of high-quality client and family-centred services that meet service recipients’ needs, ensures their safety, and works within a quality improvement framework to identify opportunities for enhancing quality, and quantitative outcomes of the services. Key accountabilities are the following:
- Accountable for providing leadership direction, clinical and operational oversight and guidance to the program(s) in relation to the delivery of Recovery Oriented, Housing First, Harm Reduction and Client and Family Centered Services at Cota.
- Collaborating with Director, North York Services, and/or other members of the leadership team in relation to leading Program Development/Quality Improvement activities within the organization.
- Collaborating and continuing to build and develop the partnerships existing amongst various affiliated hospitals within the GTA, Toronto Police Service, Canadian Mental Health Association and The Access Point.
- Improving system coordination, access to care and providing a range of innovative supports designed to meet the specific needs of the diversity of clients referred to the program dependent on their unique recovery needs.
**About the Team**
- This position is initially responsible for a portfolio that includes, but may not be limited to, managing our **Access to Resources and Community Supports - Emergency Department (ARCS-ED), Access to Resources and Community Supports - Inpatient Psychiatric Unit (ARCS-IP), Mobile Crisis Intervention Team Case Management Program (MCIT CMP)**. Each program operates as a hospital diversion program and currently includes management of interdisciplinary teams comprised of Intake Case Manager, Mental Health Case Managers, Addiction Specialist Case Managers, Nurse Case Managers and Peer Support Specialist.
**Primary Responsibilities**:
- Oversee the recruitment, training, supervision and performance management of staff and provide administrative and clinical support to frontline service providers.
- Establish new and ongoing positive relationships with community partners and external stakeholders within the North-West Toronto Ontario Health Team (NW-OHT).
- Represent Cota at all external stakeholders, committees, groups and/or tables.
- Lead and develop a program evaluation.
- Collect and analyze data based on the goals and outcome measures of the program, and specifically for reporting requirements to funders
- Identify and implement quality improvement initiatives as required.
- Provide oversight of the management of program budgets with internal support.
- Support a high-functioning team, manage staff wellness and performance.
- Schedule staff and conduct regular performance evaluations of team members.
- Play a lead role in developing standards/guidelines and providing education of program specific operating standards/guideline manuals
