Madison Community Services (Madison) is a non-profit agency that provides case management, supportive housing, and other support services through advocacy and education to individuals experiencing mental health and addictions challenges

The Program Manager is responsible for managing Madison’s Toronto Central LHIN Funded Case Management Program and the Recovery during Immigration and Settlement (RISE) program which is funded by Immigration, Refugees, and Citizenship Canada. The Program Manager provides administrative support and regular supervision to Mental Health Case Managers, as well as documenting relevant program activities and outcomes for periodic reports. In consultation with the Senior Manager of Case Management Programs, the Program Manager is responsible for ensuring program targets are met and monitoring the delivery of both the TCLHIN and RISE Case Management programs, services, and supports provided by staff.
**OVERVIEW**
- Enter, plan, collect, analyze and report on data related to QI initiatives and outcome measurements.
- Support leadership in planning and implementing QI projects and initiatives that are in line with strategic planning.
- Identify variations or trends in data, and provide recommendations for improvement.
- Monitor effects and progress of QI projects through reporting and statistics.
- Operate and maintain a variety of databases (i.e. Roxy, iCARE, etc.) related to performance and quality.
- Assess program activities in accordance with requirements to be set out by respective funders.
- Conduct surveys to measure outcomes of clients in priority areas determined by respective funders.
- Gather statistics and data in written reports and prepare presentations to the quality improvement team.
- Help coordinate documentation and survey preparation.
- Assist with coordination of other meetings and duties as assigned.
- Program / client record management and filing system creation.
**DUTIES, RESPONSIBILITIES, AND RELATED TASKS**
1. Organizational and Management
- Provides leadership, direction, and management to staff in the Case Management programs to ensure efficient delivery of services.
- Ensures staff operate within the policies and values of Madison, as well as within relevant legislation, standards and guidelines; makes certain that approved policies, procedures and practices are communicated, implemented and managed.
- Contributes a service perspective to the development of Madison’s mission and values; ensures that the short and long-term goals and targets set by funders within the program are met.
- Monitors progress in meeting client and service objectives, and takes action as required; informs the Senior Manager (Case Management Programs) of any matters related to programs, services, and supports that may affect the organization’s planning priorities, goals, and strategies.
2. Program and Services
- Provides leadership and support in the design and delivery of individual client case management services; oversees the intake process and decisions, as well as the development of individual support plans; ensures the monitoring of the health and well-being by case managers of their clients and initiation of appropriate action, where required.
- Assists in the development of new programs and services in consultation with the Senior Manager (Case Management Programs) consistent with the agency’s mission, goals, and priorities, and with the needs of our clients.
- Performs on-call duties as an emergency back-up for staff (e.g. accident, injury, fire, medical complications).
- Ensures the timely assignment of intake referrals to staff.
3. Human Resources
- Responsible for assignment and assessment of appropriate workloads to Case Managers; re-assignment of clients when necessary; consideration and approval of all vacation/ leave requests and monitoring of attendance.
- Supports and enhances the internal staff community to create an environment consistent with the mission, vision, and principles.Orient new hires and coach employees as needed, correcting or directing employee behavior where required.
- In consultation with the Senior Manager of Case Management Programs, the Program Manager develops and implements on an annual basis a plan for the training and ongoing professional development of staff and provides direct staff training, as required.
4. Administration
- Identifies staff / program needs, purchases supplies and equipment for the Case Management programs, and ensures that the work environment is consistent with all Health and Safety Standards and Regulations.
- In consultation with the Senior Manager, Case Management Programs, supports the maintenance of a comprehensive and confidential system of client-related record keeping.
