**Role Overview**

**About the Team**

The Manager, Community Health Services is accountable for providing leadership in the development and delivery of high-quality client and family-centred services that meet service recipients' needs, ensures their safety, and works within a quality improvement framework to identify opportunities for enhancing quality, and quantitative outcomes of the services.

This position is initially responsible for a portfolio that includes, but may not be limited to, managing our Modular Housing Program at 321 Dovercourt Road, Toronto. This site includes 44 permanent, affordable housing units for people exiting chronic homelessness. The team comprises Supportive Housing Workers and Personal Support Service Workers who provide the 24/7 onsite supports.

This position will work very closely with Dovercourt’s Supervisor in a collaborative manner to provide adequate support to the team, and ensure smooth operations of the Supportive Housing program.

Key accountabilities are the following:
Accountable for providing leadership direction, clinical and operational oversight and guidance to the program(s) in relation to the delivery of Recovery Oriented, Housing First, Harm Reduction and Client and Family Centered Services at Cota.

Collaborating with Director, Community Health Services, and/or other members of the leadership team in relation to leading Program Development/Quality Improvement activities within the organization

Collaborating and continuing to build and develop the partnerships with The Neighbourhood Group, which is the housing operator, while Cota provides the 24/7 onsite supports that are client-centred and solution-focused.

Improving system coordination, and providing a range of innovative supports designed to meet the specific needs of the service users, including support to maintain stable housing, crisis intervention, goal attainment, and community integration.

**Primary Responsibilities**:

- Oversee the recruitment, training, supervision of staff and provide administrative and clinical support to frontline service providers.
- Establish new and ongoing positive relationships with community partners and external stakeholders within the Mid-West Toronto Ontario Health Team (MWT-OHT).
- Represent Cota at all external stakeholders, committees, groups and/or tables.
- Lead and develop a program evaluation.
- Collect and analyze data based on the goals of the program.
- Identify and implement quality improvement initiatives as required.
- Provide oversight of the management of program budgets with internal support.
- Support a high-functioning team, manage staff wellness and performance.
