**POSITION**:Social Service Worker

**STATUS**:Full Time - Contract

**HOURS OF WORK**:37.5 hours per week

**LOCATION**:Home office is Mallorytown with travel to Brockville

**HOURS OF OPERATION**:Weekdays

**REPORTS TO**:Clinical Program Manager

**UNION ASSOCIATION**:OPSEU

**Summary of Job**:
The Registered Social Service Worker (RSSW) will provide social service support and assistance, resourcing, system navigation, and linkages to referred individuals aged 16 years and older who are referred for assistance. This is an excellent opportunity to work in an interdisciplinary team environment within a Family Health Team model of primary care.

**Qualifications and Responsibilities**
- Current registration and a member in good standing with the Ontario College of Social Workers and Social Service Workers
- Satisfactory Vulnerable Sector Check required
- Strong computer skills
- Ability to work well independently
- Collaborate effectively with physicians, psychiatrists and other team members in a shared care model
- Strong interpersonal and communication skills, both written and verbal
- Relevant work / placement experience in the healthcare field / community health team settings
- Experience working with individuals with complex needs
- Demonstrate sensitivity to diversity, whether related to race, religion, culture, sexual orientation, and/or gender identity
- Satisfactory vulnerable Sector Check required
- A valid driver’s license and the use of a vehicle along with $2 million dollar vehicle liability insurance
- Fully vaccinated against COVID-19 and provide proof of vaccination as a condition of employment.
