Job Summary: Community Housing Support Worker provides housing-related case management services to outreach clients, promoting long-term housing stability through comprehensive case development and management.

Key Responsibilities:

  • Develop and manage client case plans to promote long-term housing stability
  • Collaborate with partner agencies and community organizations to support clients' housing stability
  • Respond to issues that may impact a tenant's housing status, such as damages to a unit or conflicts with landlords
  • Maintain successful relationships with landlords, including coordinating move-outs, storage, clean-up, and damage repairs

Case Management Services:

  • Initiate a therapeutic relationship with new clients and assist them in securing permanent private market housing
  • Identify participant strengths and barriers to stability and assist participants in setting goals
  • Develop individualized case/service plans, including advocating for medical, financial, and employment benefits as needed
  • Facilitate weekly support meetings with clients and provide ongoing support based on need
  • Provide or facilitate training and education for Activities of Daily Living (ADL), life skills, financial literacy, etc.

Collaboration and Advocacy:

  • Participate in Community Intake Meetings to assess new referrals and collaborate on initiatives
  • Provide referrals to community resources, agencies, or government ministries as dictated by needs assessment
  • Develop viable options for accommodation and provide emergency referrals to appropriate services
  • Liaise with private landlords to promote and secure housing for individuals living with mental health and/or addictions

Relationship Building and Conflict Management:

  • Assess clients' stability in housing placements through regular and ongoing contact with landlords
  • MEDIATE complex issues between landlords, other community service providers, stakeholders, and clients
  • Respond quickly and effectively to tenancy issues, such as damages to the unit or problems with hygiene
  • Coordinate and participate in client move-out activities and coordinate repairs and remediation of units with landlords

Requirements:

  • Diploma in Social Work or Psychology, or Counselling, Mental Health, or Addictions Worker qualifications or other related social services field from college or university
  • Conflict resolution/crisis prevention training and experience required
  • Minimum of one (1) year's recent related experience

Knowledge, Skills, and Abilities:

  • Demonstrated knowledge of the impacts of addiction, mental health, homelessness, and poverty
  • Demonstrated ability to work with individuals who have multiple barriers to stable housing
  • Demonstrated ability to work independently and as part of an integrated community team
  • Knowledge of local community resources and the psychosocial rehabilitation model
  • Awareness of harm reduction and 'Housing First' theory and practice
