Position Overview: As a Home Care Nurse at Home Instead, you will play a crucial role in delivering exceptional nursing care to our clients in their homes. You will work collaboratively with caregivers, families, and healthcare professionals to ensure that our clients receive comprehensive and compassionate care tailored to their individual needs.

Key Responsibilities:

Patient Care:


Provide direct nursing care, including mediation administration,wound care, and monitoring of vital signs.


Conduct regular health assessments and update care plans as necessary.


Assist with personal care with personal care tasks, such as bathing, dressing, and grooming.

Collaboration:


Coordinate with caregivers, physicians, and other healthcare professionals to ensure continuity of care.


Educate clients and their families on health management and care techniques.


Act as a liaison between clients, families, and healthcare providers to communicate changes in health status and care needs.

Documentation:


Maintain accurate and timely documentation of all nursing care provided.


Ensure compliance with all regulatory requirements and company policies.

Support and Training:


Provide training and support to caregivers to ensure they are equipped to deliver high- quality care.

Qualifications:

Education and Licensure:


Current and valid Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the province of BC.


CPR and First Aid certification.

Experience:


Minimum of 1-2 years of nursing experience, preferably in home health care or a related field.


Experience in geriatric care and chronic disease management is highly desirable.

Skills:


Strong clinical skills and the ability to perform a variety of nursing procedures.


Excellent communication and interpersonal skills.


Ability to work independently and as a part of a team.


Compassionate, patient and empathetic demeanor.

Other Requirements:


Valid driver's license, car insurance, and reliable transportation.


Ability to travel to clients homes within the designated service area.


Flexible schedule to accommodate the needs of the clients, including evenings and weekends as necessary.
