DETAILED JOB DESCRIPTION
Services without coordination can be confusing and counterproductive - that's why we have a Service Coordinator. Submit your application to join a new program that focuses on preparing job seekers enter into the Human Services Sector.
This new program targets Albertans furthest from the labour market, including individuals experiencing or at risk of long-term unemployment due to factors such as limited work experience, low education or skills, disabilities, language barriers, systemic discrimination, or lack of community connection.
What will you do?
- Provide orientation to Prospect and program services, assess client goals and needs, schedule – coordinate and carry-out intake and planning meetings with clients and members of their support network, arrange for accommodations or assistive technologies where appropriate, determine acceptance of clients into service, develop employability assessments, service plans, employment profiles and client schedules.
- Act as the central point of contact for the client support network, from intake to completion of service.
- Collaborate with employer partners to help source, train, and retain quality employees within the three main streams of the program:
- Disability Services
- Continuing Care
- Housing
- Support other Prospect direct service positions in working directly with Prospect clients; providing support in order to help expand employability skills development.
- Maintain knowledge of supports and resources within the community, including financial supports, funders, mental health service providers, supports for persons with developmental disabilities, crisis supports, behavioral supports, counseling supports and education and training supports.
What does it take to do this job?
- Post-secondary education in a related field and 2 to 3 years' experience with case management/service coordination in a similar industry or environment.
- Exceptional oral and written communication skills.
- Excellent organizational and analytical skills.
- Demonstrated proficiency with Microsoft Office.
- Prior experience with intake processes, employability assessments, accommodations, and service plans would be considered strong assets.
Further Notes:
- This is currently a hybrid role, offering the flexibility to work from home part of the time while also connecting with others through in-office collaboration.
- Those with a combination of post-secondary education along with professional experience may be considered.
- A reliable vehicle and valid driver's license is required for business travel.
