We serve a remarkable community of individuals with diverse backgrounds and experiences. Since founding in 1987 as part of the Welcome Home program, we've been dedicated to supporting adults with intellectual disabilities as they transition from institutional settings to vibrant community living. We provide 24-hour support to individuals with Autism Spectrum Disorder, FASD, Mental Health challenges, problematic substance use, and behavioral and sensory challenges, ensuring each person receives the care, respect, and opportunities they deserve. Case Manager Job Summary:
The Independent Living Case Manager is responsible for managing their assigned caseload to ensure team members provide the required support for people living independently in the community. The Case Manager works collaboratively to create case plans and initiatives for those under their care.
The Case Manager is Responsible for:
- Managing a caseload of individuals with a variety of needs, assessment of needs, development & implementation of support plans.
- Working collaboratively with team members, families, & collaterals to help meet goals.
- Effective communication with all stakeholders.
- Providing leadership, training, supervision, guidance & direction for case workers & outreach support mentors.
- High work standards, including a fast-paced environment, strong attention to detail, strong time management skills, and organization.
A high degree of professionalism, including maintaining boundaries, confidentiality, and a respectful workplace.
Administrative skills are a priority for this role. Competency with computer programs and the ability to complete administrative tasks quickly and efficiently are critical.
- On Call Rotation.
Qualifications:
- 2 – 3 years of Case Management experience or a similar role preferred.
- University education in social services or equivalent.
- Minimum 3-5 years of experience working in the vulnerable sector in an outreach capacity.
- Experience supporting individuals with a range of complex needs, including but not limited to Autism Spectrum Disorder, FASD, Mental Health challenges, problematic substance use, and behavioral and sensory challenges.
- Strong relationship-building skills.
- Demonstrated language, writing, and comprehension skills.
- Strong problem-solving skills.
- Proficiency in the use of computers.
- Possesses a safe & reliable vehicle, proof of insurance & satisfactory driver abstracts.
- Must have and maintain a Full Class 5 Manitoba Driver's License.
- Current and valid Emergency First Aid and CPR Certification.
- The ability to work a flexible schedule is required.
- Ability to collaborate with and support other team members and partners to advance shared goals and projects; comfortable working in a collaborative and shared leadership context.
- Ability to work in a multicultural environment.
- Must be eligible to work in Canada.
Compensation & Benefits:
The Case Manager's wage starts at $26.35 an hour.
Simaril offers a generous benefits package that includes medical, dental, vision care, pension, and paid time off.
Hours and Working Conditions:
This is a full-time position with flexible standard work hours 9:30 a.m. to 5:30 p.m., Monday through Friday. To provide additional support outside these hours, you may be required to work some overtime hours with paid compensation.
This position will combine working in a fast-paced office and working in the field to provide ongoing support to the outreach team and the people supported.
Preference will be given to those who reside within the city limits.
