Job Summary
The case management team is a key connection to outside organizations and the participants trying to access those services. They work with individuals who access MSP services to identify, develop, implement and assess the self-identified goals of participants. Case managers work from a housing first, harm reduction and trauma informed lens to support people to find and maintain housing and to connect to primary care, income, addictions & mental health supports to break down barriers and advocate for their participants. Relationship building is the key function of this team.
Referrals into this team come from support staff within MSP or external agencies. They are reviewed by the Program Manager and assigned to a case manager based on the space within a caseload, the acuity of the participant and their immediate needs. Once assigned, the case manager meets regularly with participants within MSP and the community.
Key Responsibilities
General Case Manager Duties:
- Provide on-going case management support to assess progress and ensure treatment plan
- Develop and coordinate individualized care plans addressing short-term and long-term goals in collaboration with participants, interdisciplinary team members and external partners
- Assess an unsheltered person’s needs in terms of moving towards housing Identify in a proactive manner, barriers that restrict and develop strategies that facilitate the participants’ ability to meet identified goals
- Create and maintain a professional relationship with community members, including boundary setting
- Keep accurate, continuous, legible and signed records including progress notes, checklists, correspondence, journals for individual and collective access in accordance with established protocols pertaining to access to and disclosure of casework files
- Work with Involuntary Participants and Participants not capable of consent
- planning process and intervention with participant
- Identify and provide information and resources to participants including:
- Community resources
- Rent and income assistance
- Mental health resources
- Indigenous organizations
- Establish partnerships with landlords and service agencies, and advocate on behalf of participants
- Identify symptoms of suicide ideation and offer resources and support
- Create environments of cultural safety for participants
- Meet with participants both on-site and in community
- Monitor, evaluate and communicate progress of participants with the participants themselves, team members and managers on a regular basis
- Maintain accurate and confidential electronic and file records, including statistical data
- Conduct crisis intervention as needed
- Attend program-related community, coalition, and committee meetings
Qualifications, Skills and Abilities
- Bachelor's Degree in a relevant field. Experience working frontline with people who are unsheltered, lived experience and/or a combination of education and experience will be considered in lieu of a degree.
- Demonstrated knowledge of case management principles and processes - eligibility, assessment, planning, implementation, evaluating, transitioning, documentation and file management
- Demonstrated experience developing and implementing care plans that promote positive health outcomes
- Experience creating and maintaining case plans, progress notes and other necessary documentation
- Experience working with Indigenous organization and/or communities
- Experience in working with culturally diverse populations
- Computer proficiency in Microsoft Office and any other database software(s) used to track service delivery
- Non-violent crisis intervention (NVCI), mental health first aid, suicide intervention, and/or trauma informed training considered an asset
- An understanding of and ability to work within the frameworks of client-centered care, self-determination, strength based approach, and trauma informed care
- Excellent interpersonal skills and the ability to form relationships with community members including a proven ability to communicate appropriate professional boundaries
- Motivational interviewing skills
- Excellent verbal and written communication skills
- Knowledge of Indigenous History and Colonial History and it’s impact on the reality of Indigenous people in Winnipeg.
- Knowledge of TRC Calls to Action, MMIWG Call to Justice, and UNDRIP
- Indigenous language skills are considered an asset
- Knowledge of community resources available throughout Manitoba
- Knowledge of addictions, mental health diagnoses, and co-occurring disorders
- Knowledge and ability to adhere to confidentiality standards and legislation
- Ability to manage, assess, and work with challenging behaviors
- Ability to work independently, under pressure, in a team or independent setting
- Ability to effectively resolve and cope with conflict and/or cri
