**CASE MANAGER**
**Pikangikum Allied Health Services (PAHS)**
**Position Description**:
Under the direction of the Pikangikum Allied Health Manager and the Pikangikum Education Authority (PEA), the Case Manager will work to help organize and deliver case management services in the Pikangikum Allied Health Services (PAHS) program.
The PAHS program is a collaboration between PEA and Pikangikum Health. This position will be located from a primary office in Winnipeg and travel to the Pikangikum First Nation is required one week per month or as needed for assessment, monitoring of treatment plans, provision of services, interdisciplinary team meetings, staff training and mentorship to other staff.
**Responsibilities will Include**:
- Screen all program referrals and assign cases
- Manage program waitlists
- Conduct comprehensive assessments both formally and informally with children and youth with academic, behavioral, cognitive, social or emotional challenges
- Coordinate reports and treatment plans with clear goals to help further education outcomes and healthy functioning
- Liaise with other disciplines to ensure wholistic case planning
- Lead interdisciplinary team meetings on case plans
- Work with children, families and educators to identify the resources needed to address the gaps
- Engage with both community providers and external organizations to build and nurture successful working partnerships
- Responsible for the overall file management of all the cases in Allied Health Program
- Provide navigation support to appropriate programs, departments and agencies and coordinate referrals as needed
- Work with professional and para-professional staff to support in a responsive and coordinated manner
- Ensure consent forms and documentation are completed as required
- Ensure children and families receive high quality culturally appropriate services and supports at all levels of care
- Ensure assessments are consistent with the child’s most natural learning environment including home assessments
- Provide education at an appropriate level to school personnel and parents/caregivers that would assist them in developing a greater understanding of their child’s functioning
- Provide services in person (1 week per month) and via teleconferencing and with the use of video/web technology for the remainder of the time
- Work closely with the other members of the Pikangikum Allied Health team and the Learning Resource Team at Eenchokay Birchstick School and other treatment providers on interventions and goals
- Follow up with Pikangikum Allied Health Team members and the Learning Resource Team at Eenchokay Birchstick School to ensure progression of the treatment goals.
- Participate in the development of the PAHS program, forms, guidelines, procedures and policies as it relates to the provision of services and case management
- Develop training tools and deliver training workshops and deliver training workshops
- Attend professional development opportunities to support the position and disseminate knowledge to other PAHS team members
- Ensure that proper case records, assessment reports and statistical records and all other pertinent data to each case is maintained and adhere to confidentiality guidelines
- Be available for other duties as identified in the ongoing development of the PAHS program.
- Work under the policies of the Pikangikum Education Authority and the Pikangikum Allied Health Services program
**Qualifications**:
- Experience in delivery of case management services to children and youth in a school setting
- Bachelor or Masters Degree in Social Work from a recognized university
- Good knowledge of the impact of pediatric conditions that can adversely affect learning and development
- Knowledge of the impacts of colonialism, Residential Schools, intergenerational trauma and systemic racism on First Nation education and children
- Experience working in a collaborative team environment
- Excellent organizational and time-management skills as well as strong independent/team working skills.
- Excellent written and oral communication and well-developed interpersonal skills.
- Demonstrated ability to organize time, establish and meet deadlines, and take initiative to solve complex problems in collaboration with colleagues, teaching staff, school administrators and paraprofessional staff.
- Ability to prioritize multiple tasks
- Ability to communicate with children, parents/caregivers and other professionals
- Excellent interpersonal and cross-cultural skills to work in a First Nations environment
- Knowledge of Jordan’s Principle federal programming and services covered
- Strong technical/computer skills
**_* The PEA offers a competitive compensation and benefits package based on qualifications and experience. Salary range is $80,000-$100,000 annually_**
