Responsibilities

  • Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
  • Provide medication administration and monitor vital signs in accordance with care plans
  • Support clients with memory care needs such as Alzheimer's or dementia, ensuring a safe environment
  • Prepare nutritious meals tailored to individual dietary requirements and assist with meal planning
  • Perform light housekeeping tasks including cleaning and maintaining a safe living environment
  • Observe and document patient conditions, reporting any changes or concerns promptly
  • Use specialized equipment such as Hoyer lifts or resident lifts to assist with mobility needs
  • Implement care plans developed by healthcare professionals, ensuring adherence to HIPAA regulations
  • Engage in patient monitoring to ensure safety and well-being during daily activities
  • Support clients with developmental disabilities or autism through direct support and social engagement
  • Maintain accurate records of patient care activities as required by healthcare standards

Skills

  • Proven experience in home & community care, assisted living, or long-term care environments
  • Knowledge of patient care practices including medication administration, vital signs monitoring, and dementia or Alzheimer's care
  • Certifications such as First Aid, CPR, and HIPAA compliance are highly desirable
  • Ability to work independently while collaborating effectively with healthcare teams
  • Strong observational skills for patient monitoring and safety assessment
  • Compassionate approach with excellent communication skills for working with clients and their families
  • Physical ability to perform heavy lifting when necessary and assist with mobility using specialized equipment
  • Experience working with people with disabilities or developmental challenges is preferred
  • Knowledge of social work principles related to elder or disability care is a plus
