Home Support Worker

**Job Summary**
**Duties**
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and record vital signs such as blood pressure, pulse, and temperature
- Administer medications following care plans and HIPAA regulations
- Prepare nutritious meals tailored to dietary needs and preferences
- Provide light housekeeping, cleaning, and laundry to maintain a safe environment
- Support clients with memory care needs, including dementia or Alzheimer's care
- Use assistive devices such as Hoyer lifts or resident lifts for safe transfers
- Observe and report any changes in client health or behavior to healthcare professionals
- Assist with transportation to appointments or community activities when required
- Follow established care plans and protocols for patient safety and well-being
**Requirements**:
- Prior experience in home & community care, assisted living, nursing homes, or long-term care preferred
- Knowledge of patient care techniques including CPR, first aid, and medication administration
- Experience working with individuals with developmental disabilities, autism, or dementia is highly desirable
- Ability to work independently while maintaining safety standards and HIPAA compliance
- Strong communication skills for effective interaction with clients, families, and healthcare teams
- Physical ability to perform heavy lifting and assist with mobility support as needed
- Valid driver’s license preferred for transportation duties
- Certification in CPR or first aid is a plus but not mandatory; willingness to obtain certification required for some roles
