We are seeking a skilled Client Care Coordinator to join our team as a subcontractor to support our mission of improving client outcomes and delivering exceptional in-home care experiences.
Position Overview:
As a Client Care Coordinator Subcontractor, you will be instrumental in ensuring seamless client interactions, coordinating personalized care plans, and upholding the highest standards of communication and service. This role demands strong organizational skills, a compassionate client-focused approach, and the ability to work independently. You will collaborate with our caregivers, clients, and families to deliver exceptional care under a one-year renewable contract, managing your own schedule and deliverables as an independent contractor.
Key Responsibilities:
- Act as the primary point of contact for clients and families, addressing inquiries, scheduling care services, and providing timely updates.
- Coordinate in-home care services, including developing and managing customized care plans, scheduling caregivers, and ensuring consistent follow-up care.
- Maintain accurate, up-to-date client records using electronic medical records (EMR) systems and/or CRM software.
- Communicate effectively with clients, caregivers, healthcare professionals, and external partners to ensure seamless care delivery.
- Resolve client concerns promptly, escalating complex issues to management while maintaining high satisfaction levels.
- Ensure compliance with Alberta healthcare regulations and CCAHS policies to meet quality and safety standards.
- Prepare and submit timely reports and invoices for billable hours, adhering to agreed-upon deliverables.
Qualifications:
- Experience: Minimum 1-3 years in client care coordination, patient care, or a related role within healthcare or home care services.
- Skills:
- Strong organizational and time-management abilities.
- Excellent verbal and written communication skills.
- Proficiency in EMR systems (e.g., ClearCare, WellSky), CRM software, Microsoft Office, and Google Suite.
- Knowledge of Alberta healthcare regulations and home care standards is an asset.
- Bilingualism (English/French or other languages) is an asset.
- Education: High school diploma or equivalent; post-secondary education in healthcare, nursing, social work, or administration is preferred.
- Attributes: Compassionate, detail-oriented, proactive, and able to work independently with a client-centered approach.
- Technical Requirements: Reliable internet, computer, and phone for remote work.
- Subcontractor Requirements: Must provide own equipment, maintain professional liability insurance, and handle self-employment taxes.
Compensation:
- Hourly rate of C$27 - C$35, based on experience, qualifications, and complexity of duties.
- Paid bi-weekly upon submission of approved invoices under a one-year renewable contract.
- No employee benefits provided, as this is a subcontractor position.
Work Schedule:
- Flexible, approximately 20-30 hours per week, with availability for occasional on-call support.
- Primarily remote with occasional in-person meetings or client visits in Calgary as needed.
- Billable hours limited to agreed-upon tasks (e.g., client interactions, care coordination, documentation; travel or non-client-facing research time excluded unless approved).
