Position Summary
The Case Management Coordinator, Social Worker, is responsible for providing clinical support, supervision and direction to the Community Support Workers in the Case Management Program, Community Mental Health Services. The Case Management Coordinator is also responsible for managing program service wait lists, and provides leadership support to the CMHS team, Quality Experience Lead (QEL), & Program Manager to contribute towards program review and development. This role acts as a proactive liaison within the CMHS team, the hospital’s services, inpatient acute mental health unit, acute out-patient mental health and day hospital program, Mental Health Crisis Line, family physicians, and community groups and organizations (e.g. police, shelters, churches, schools, and agencies), to facilitate appropriate use and promotion of services.
**PRIMARY RESPONSIBILITIES**:
- To provide daily operational and clinical support to the Community Support Workers in the Case Management Program, including at The Meeting Place.
- Assessing, assigning and coordinating primary case management client assignments and supportive case management duties, including outreach activities, The Meeting Place programs and support activities.
- Maintaining and communicating the Case Management team calendar, in collaboration with the Community Support Workers, ensuring primary case management and supportive case management appointments and duties are assigned in an equitable and ethical manner.
- To provide clinical support to the Community Support Worker team, by fostering identification of strengths, opportunities for education and professional growth, actions and plans to support professional development.
- Collaborating with the Case Management and Counselling & Treatment teams to support program and client specific goals and activities as required.
- Provide support to the Case Management Team by assisting with a range of clinical services in delivery of client, outreach, education or advocacy services, and collateral contacts with family members and other professionals, in accordance with the standards of practice of the Ontario College of Social Workers and Social Service Workers.
- Support referrals of clients in the Case Management program to internal, external, community or government partners and programs.
- Assist with the management of the CMHS program services wait list(s), by assisting with screening intakes, providing monthly contacts to clients on the wait list, triaging referrals, completing referrals to internal, external, community and government partners, and communicating assessments to appropriate team member(s).
- The position requires considerable independent decision-making and professional judgement on matters pertaining to problem assessment, methods of intervention, and client safety in a multi-disciplinary environment.
- To liaise with physicians, professional agencies/services in the community, hospital staff, and family members, wherever appropriate, for the purpose of consolidating and coordinating client treatment.
- To maintain clinical records of course of treatment in accordance with the standards of practice of the discipline, and to assume full responsibility for compliance with this in accordance with the standards of practice of the Ontario College of Social Workers and Social Service Workers.
- To maintain adequate and accurate statistical data as required, and to participate in survey/research projects.
- To assume responsibility for their own continuing professional development, and to document this activity in an annual Professional Development Plan.
- To participate with other staff in monthly (or ad hoc) business meetings for the review of policies and procedures, and on-going program development and evaluation.
- To develop specialization(s) in skills and competencies, and take leadership in promoting this within the program.
- To provide support and supervision to students, volunteers and clients who may be involved in the delivery of services within the program.
