Job Description:
Position Overview:
Reporting to the Stroke Program Manager, the Stroke Best Practice Coordinator will be the co-lead for Stroke Distinction award process through Accreditation Canada; provides clinical support and intervention with stroke patients across SHN - based on need, complexity and specific program requirements; as well as provides evidence-based support for staff (nursing and other disciplines) for knowledge transfer through education and learning opportunities; mentoring and coaching and working alongside with staff for just in time learning.
The Stroke Best Practice Coordinator will provide internal expertise through change management to enable the sustainability of best practices and quality improvement for the Stroke program. This coordinator will also work collaboratively with other similar positions within the Toronto Stroke Network (TSN) region to develop standardized care practices, policies and procedures as it relates to the delivery of stroke care in all continuum of care - hyperacute, acute, inpatient rehab, outpatient rehab & the stroke clinic.
The Stroke Best Practice coordinator provides support to the Program Manager in the development and roll out of TSN regional initiatives, and leadership.
The incumbent must commit to fostering a safe and healthy environment for patients, staff and others, ensuring compliance with health and safety policies and legislation, and the code of conduct of the hospital.

**Requirements**:
- Bachelor of Science in Nursing
- Current registration with College of Nurses of Ontario
- Minimum three years of clinical experience related to the care of patients experiencing stroke
- Proven flexibility and ability to adapt positively to change
- Demonstrated commitment to continued education and improvement
- Excellent interpersonal, leadership, and team building skills
- Demonstrated experience with best practices & TSN stroke standards of care
- Demonstrated leadership skills, ability to mentor staff and facilitate program and staff development.
- Excellent verbal, written and presentation skills
- Demonstrated knowledge and skills in analytical, critical thinking, decision making and problem solving
- Demonstrated knowledge of data management and analysis
- Act as primary contact for all admitted stroke patients at all SHN sites
- Ability to time manage, prioritize competing priorities, work independently and in a group setting
- Commitment to and understanding of the mission and values of SHN
Asset:
- Master’s Degree completed or in progress in a health discipline
- CNA certification - Neurosciences nursing
- Ability to speak another language
