**Our Team**:
As an integral member of Community Programs within Seniors’ Services, you will be supporting clients who are receiving in-home community supports in order to continue to live as independent as possible. You will work collaboratively with internal and external stakeholders to ensure client’s needs are met. You will support and liaise with other program areas within Community Programs to ensure all programs are thriving and delivery quality services to clients.
**The Opportunity**:
As Coordinator, Homemaking Services you will have the responsibility of determining financial and functional eligibility for homemaking services through assessing applicants' financial and health-related information, strengths, and deficits in instrumental activities of daily living, and current informal and formal supports. You will use your skills and experience to establish and monitor service plans and ensure they are appropriate to individual client needs. You will have the opportunity to provide clients with information on community resources. You will communicate and collaborate with family and other supports, with clients’ consent. You will be responsible for responding to requests for information regarding the program. You will review and screen new referrals and assess clients for priority status. You will also monitor, maintain, and update the waitlist.
You will have the responsibility of providing oversight, guidance, and support to contracted agencies to ensure client needs are met.
You will have the opportunity to conduct community home visits to assess clients’ financial and functional eligibility, homemaking needs, and suitability/safety of home environments. You will use your expertise to determine eligibility based on legislation, standards, regulations, and policies. You will refer ineligible clients to other community resources.
You will develop, implement, monitor, and reassess individualized service plans in collaboration with clients, care partners, contracted service providers, and other health care and support services.
You will use your skills and experience to maintain program data and documentation standards consistent with professional guidelines, policies, and legislation.
You will participate in program development, evaluation, quality improvement activities, and professional development.
You will allocate program resources and monitor the program budget.
You will support other members of the Community Programs team as required.
Performs related duties as required.
Knowledge of health and community resources and service delivery, assessment and referral, supportive counselling, and crisis intervention, normally acquired through a bachelor’s degree in a health related or social services discipline, plus related experience in a community-based mental health or older adult environment.
Knowledge of and ability to comply with policies, procedures, collective agreements, and related legislation (e.g., long term care, health and safety, privacy, social assistance).
Assessment, analytical, interviewing, organizational, and continuous improvement skills to perform functional, environmental, and risk assessments; coordinate and monitor service delivery; and recommend program improvements.
Communication, human relations, and leadership skills to resolve conflicts and mediate disputes; interview and advise clients and their families (including negotiating service plans); respond to complaints and inquiries; and participate as an effective team member.
Ability to write reports, minutes, correspondence to families and agencies, client progress notes, and policies and procedures. Ability to read and understand policies, procedures, journals, newsletters, correspondence, charts, reports, client files.
Computer skills with ability to use software such as Microsoft Office, and client health information databases and systems.
Must have a tuberculosis (TB) test and all required immunizations.
Must provide an acceptable vulnerable sector police check.
Ability to work flexible hours and work variable shifts, including evenings, holidays, and weekends, as required.
Ability to travel within Waterloo Region.
