The Clinical Services Pathway Coordinator (CSPC) plays an integral role in monitoring and optimizing timely and appropriate access to clinical services. The CSPC will provide appropriate case assignment and access to service/information to children and families who are waiting for service delivery or transitioning from service. The CSPC will work collaboratively with all managers across the Clinical Services Division to ensure a seamless, integrated approach to service delivery from point of referral. The Coordinator will also liaise with referral sources and other community service providers with respect to referral status and transitions.

Key responsibilities include:
**Waitlist Management and Case Assignment**
- Monitor waitlists across all portfolios.
- Make timely decisions on referrals and case assignments, based on policies, procedures and risk, including HCCSS referrals.
- Monitor and assess clinician workload, productivity and workload capacity.
- Assign clients to appropriate clinician(s) including assessing for prioritization, urgency, clinician availability and geographical need (use of satellites sites).
- Identify opportunities for grouping clients for services and the nature of the groups.
- Provide updates to Clinical Managers regarding case assignments.
- Review and identify productivity and workload issues of clinicians and inform clinical manager of actual or potential performance concerns. Provide input into corrective actions, if appropriate.

**Facilitate Access to Information, Services and Supports**
- Respond to enquiries from families who are waiting for service initiation, and update client records based on clinical assessment/judgement and prioritization.
- Review client needs and act as a point of contact for clients waiting for service initiation including providing information about appropriate resources while waiting for service.
- Respond to other health care providers and community agencies to review shared clients waiting for service initiation.
- Triage issues that require Manager input/resolution.
- Document concerns/complaints.
- Respond to routine service pathway determination questions from program staff as a first response in collaboration with the Clinical Practice Facilitator

**Support Transition Processes**
- Facilitate the transition to school process including aggregating data about clients across the division to share with school boards and community partners and supporting staff in the appropriate processes for transition to school.

**Quality Improvement and Program Development**
- Attend Clinical Leadership meetings to discuss waitlist management pressures and improvement strategies.
- Inform the Clinical Leadership Team of challenges related to service access and pathways identified by families, referral sources and clinical staff.
- Identify and communicate to Director divisional trends in waitlist data that may require a human resource review.
- Develop universal resources and supports across the division to support program delivery.
- Monitor the assignment of clients to ensure compliance with prescribed standards and clinical policy, including prioritization of urgent referrals and accurate service pathways.
- Act as a resource and support to staff when determining appropriateness for referral to service and respective service pathways.

**Data Integrity**
- Contribute to data integrity related to the wait list and case assignments. Update wait list data as appropriate within the CSPC role.
- Identify factors, trends and strategies to support data accuracy.
- Support the development of strategies to improve wait list accuracy, in collaboration with the Clinical Leadership team.

**Other**
- Other specific duties/projects as may be assigned by Clinical Leadership.

**Health and Safety**

The Clinical Services Pathway Coordinator will comply with the duties assigned to Workers as per Section 28 of the Ontario Occupational Health and Safety Act.
