Client Care Coordinator

We are seeking a compassionate and highly organized Client Care Coordinator to support individuals recovering from motor vehicle accidents and trauma. This role is ideal for someone who values relationship-building, community collaboration, and guiding clients through challenging moments with clarity and care.
**Locations**: This position is at our Kitchener clinic location.

**Key Responsibilities**:
- Coordinate new referrals from intake through to service initiation
- Connect with clients to gather key information and provide clear, supportive explanations of the process
- Document client needs related to physical, emotional, and cognitive challenges
- Prepare and manage intake forms and digital documents, ensuring timely completion and follow-up
- Maintain ongoing communication with clients and referral partners to provide status updates
- Track the progress of each intake to ensure nothing falls through the cracks
- Build strong relationships with legal, healthcare, and community partners to support seamless client care
- Provide thoughtful, trauma-informed communication to individuals who may be overwhelmed or in distress
- Support clients in understanding next steps and available services
- Contribute to a high-quality, client-centered experience from first contact to service launch
**Need HCAI and MVA Experience**
**Who You Are**:
- An excellent communicator who is patient, empathetic, and detail-oriented
- Passionate about community connection and building strong partnerships across teams
- Able to manage a high volume of tasks with professionalism and follow-through
- Confident working with individuals recovering from injury or trauma
- Experienced in customer service, healthcare, or rehabilitation settings (asset)

**Salary Range**: $50,000
Given applicants will be of variable backgrounds for this position, compensation will be discussed at the time of interviewing based on skill level, experience and education.
