The Housing Navigator supports clients with complex needs who also have an identified housing need. The Housing Navigator is part of a network of community partners and government agencies working collaboratively to support individuals with complex needs. The incumbent will have a strong understanding of the underlying issues contributing to the complexity of the client base including mental health, addictions, behavioural issues, cognitive disorders, and/or concurrent disorders and factors that contribute to homelessness. The incumbent will be responsible to conduct client assessments, determine appropriate supports needed and work with clients to develop individualized case management plans.
**Duties will include but are not limited to**:
- Assessing the immediate housing needs of clients with complex needs who are referred by other government agencies and community partners in order to determine required supports and services;
- Complete assessments to determine specific issues faced by clients which are creating situations of homelessness, housing insecurity, or imminent risk of homelessness;
- Lead the development of case planning and management for complex clients;
- Develop an individualized case management plan based on the client assessment, utilizing best practices, homelessness assessment tools, understanding and identifying government and community resources available to the client;
- Work collaboratively with a variety of external organizations to assist clients to access programs and supports;
- Participate in community tables, including the case conferencing table designed to support clients of the community outreach centre;
- Work and communicate effectively with, community partners, government agencies and client supporters to provide wrap around supports to clients;
- Offer service at the Community Outreach Centre on a regular basis, meet with clients and organizations, and receive client referrals from multiple community service partners and government departments;
- Complete reports and maintain confidential records;
- Quickly and accurately assess risk associated with specific client circumstances;
- Participate in awareness building campaigns to reduce stigmas and misconceptions around homelessness; and
- Other duties as required to support the Housing Services Division.
**Minimum Qualifications**:
- Must have a university degree in social sciences, social work, business administration, or other related discipline;
- Demonstrated equivalencies will be considered;
- Considerable experience working in the field of housing, social programs, health or community development is required;
- Considerable experience working with vulnerable populations;
- Considerable experience in determining and providing case management supports;
- Experience working collaboratively with community partners and other government organizations;
- Strong technical and computer skills, strong record keeping skills;
- Excellent interpersonal skills;
- Mental health first aid and trauma and violence informed care;
- Current CPR and First Aid Certification;
- Knowledge of the Housing Corporation Act, the Rental of Residential Property Act (and successors), the health and social service systems, and current social housing trends;
- A good understanding of financial documents (RRSPs, Annuities, Assets, etc.).
**Other Qualifications**:
- Naloxone and Non-Violent Crisis Intervention Training;
- Experience in the development and implementation of assessment tools would be an asset;
- Formal case management training would be an asset;
- Access to a reliable vehicle and a valid Prince Edward Island driver’s license;
- Working knowledge of provincial housing programs and supports;
- Knowledge of local community, provincial, and federal resources related to mental health/addictions and vulnerable populations; and
- Demonstrated ability to diffuse difficult and complex situations and be able to respond appropriately while working independently
