Reporting to the Housing Program Administrator, the Outreach Worker will support tenants of the Seniors Housing Program to maintain successful housing placements by achieving and maintaining health and independence through the appropriate quality services. The incumbent will assess tenant needs, determine solutions and in collaboration with the tenant, family, and service providers support the development of a case plan to implement and monitor solutions. The incumbent will need to have a strong understanding of the seniors population, those experiencing persistent mental illnesses, addictions, behavioral issues, forensic histories, cognitive disorders, significant mental health issues and/or concurrent disorders.

**Duties will include but are not limited to**:

- Encourage and facilitate productive communication with the tenant, while recognizing complex issues of emotional and cognitive development in the individual.
- Support tenants to learn about their illness and recovery process by connecting them with appropriate services.
- Collaborating with tenants, families and service providers, develop an individualized case plan based on the tenant’s assessment; determine priorities, provide interventions, and set realistic goals.
- Provide ongoing case management, monitoring any changes in health or behavior, and making adjustments as required.
- Promote coordinated delivery of all government services in the community and maintain excellent cooperative working contacts with other departments/agencies for referring tenants and obtaining information: manage service provision in such a manner as to avoid duplication and to promote integrative team work.
- Collaborate with health care professionals and community organizations to develop and coordinate plans and offer feedback on a tenant’s progress.
- Identify resources that will support the tenant and allow opportunities for appropriate social support and development.
- Research available assessment tools, and adopt or modify as required to meet program needs.
- Receive tenant referrals from Housing Services staff; gather sufficient initial tenant information to prioritize referrals.
- Regularly review and prioritize caseload to ensure highest tenant needs, especially quickly changing conditions, are met.
- Other duties as required.

**Minimum Qualifications**:

- Must have a bachelors degree in a health related discipline or social work.
- Considerable experience working in the field of housing, health, or community services
- Considerable experience in individual and group facilitation and conflict resolution
- Demonstrated experience in case management with individuals with complex needs that includes collaboratively working with health services such as mental health, addictions, home care, adult protection
- Experience in development and implementation of assessment tools
- Experience in leading initiatives or projects involving government organizations, and community members and working collaboratively with community groups.
- Considerable experience working with the senior population and / or vulnerable sector
- Demonstrated equivalencies will be considered.
- Strong interpersonal, communication and organizational skills
- Ability to exercise diplomacy, fact and judgement in dealing with difficult and complex situations.
- Strong technical and computer skills; Microsoft Access, Excel, Power Point, MS Word
- Ability to work with a high level of autonomy with mínimal supervision
- A valid driver’s license and access to a reliable vehicle
- A good previous work and attendance record
- Current CPR and First Aid Certification
- A criminal record check and vulnerable sector check

**Other Qualifications**:

- Non Violent Crisis Intervention Training
- Working knowledge of the Mental Health Act and Housing Corporation Act
- Knowledge of community-based health and social services related to mental health/addictions and seniors’ issues
- Understanding of the needs of seniors experiencing mental illness and other senior specific conditions
- Knowledge of department’s function, priorities and policies would be an asset
- Skills at preparing, delivering, and facilitating workshops and conducting presentations
- Excellent assessment, interviewing, and negotiation skills
