**Position Summary**:
Reporting to the Director of Quality Practice Education, the Knowledge Management Coordinator supports quality healthcare for residents and families by working to ensure staff have access to policies and on-line training resources needed to support all aspects of resident care.
**Primary Role and Responsibility**:
**1. Accountabilities**:
i. Supports the development of a document management process and manages digital policy documents on the intranet ensuring that everyone has the most updated version at their disposal
ii. Communicates new and updated policies to stakeholders, including important changes to practice
iii. Generates reports on policy usage and activity, providing insights and recommendations to stakeholders
iv. Supports the development of an LMS and manages digital learning content within the LMS
vi. Trains new users on how to use the LMS, including best practices for content creation and usability
vii. Generates reports on LMS usage and activity, providing insights and recommendations to stakeholders
viii. Collaborates with cross-functional teams to ensure the LMS meets the needs of all users
ix. Establishes and maintains effective working relationships with internal and external stakeholders
x. Models dedication, commitment and resourcefulness in supporting Covenant’s fulfillment of the mission and achievement of the vision
xi. Handles conflicts with contacts in a professional and diplomatic manner
**2. Service Excellence**:

i. Inspires others to achieve the best possible care for those we serve
ii. Recommends actions to address identified gaps in quality, care, service and practice, using evidence-informed clinical practice standards
iii. Participates in development of learning programs for new and revised policies, procedures and clinical care and practice initiatives
iv. Builds relationships with, engages with and collaborates with site staff, clinical leaders and educators to modify and/or develop applicable resources for teams
**3. Team Work**: Cares for and works cooperatively with others as an effective team member. Performance Measures Include:
i. Collaborates assesses trends in research and data to support Covenant Care and Covenant Living clinical priorities
ii. Works collaboratively with stakeholders to ensure that resident safety issues are identified and addressed
**4. Pro-active Learner**: Demonstrates the vision and values of Covenant Care and Covenant Living and lives out the mission of Creating Caring Communities.
**Performance Measures include**:
i. Communicates, liaises and networks with others in health care to learn from their experiences and to optimize implementations of similar concepts within Covenant Care and Covenant Living
ii. Understands, follows and applies all Covenant Care and Covenant Living Standards of Conduct
iii. Attends and actively participates in educational opportunities and health and safety training as required
iv. Collaborates, communicates and shares knowledge and learning experiences with others
v. Demonstrates life-long commitment to learning
**5. Safety**: Contributes to a safe environment for residents, the team and self by understanding and adhering to safety guidelines and practices at all times.
**Performance Measures include**:
i. Identifies and mitigates unsafe conditions in the work environment, including immediate reporting of any unsafe conditions
ii. Strong knowledge of body mechanics
