Job Title: Registered Social Worker


About the Role:


Rehab First Inc. is a leading multidisciplinary assessment and treatment organization seeking a skilled Registered Social Worker to join our team. As a key member of our team, you will play a vital role in providing psycho-social assessments, case management, and counseling services to clients affected by serious injury and illness.


Responsibilities:


  • Conduct comprehensive psycho-social assessments to identify client needs and develop effective treatment plans
  • Assign and manage case loads, ensuring timely and effective support to clients
  • Develop and implement OCF-18 plans under Ontario's Statutory Accident Benefit regulations
  • Provide counseling services in various settings, including in-person, telephone, and cyber counseling
  • Document and report client progress and outcomes
  • Collaborate with medical and rehabilitation professionals to ensure comprehensive care

Requirements:


  • Master's degree in Social Work (MSW)
  • Current registration in good standing with the Ontario College of Social Workers and Social Service Workers
  • Experience in a healthcare setting, preferably in the community
  • Familiarity with relevant legislation, including the Health Care Consent Act and Mental Health Act
  • Excellent communication, problem-solving, time management, and documentation skills
  • Self-motivated and able to manage a busy and varied caseload
  • Valid Driver's License and access to a reliable vehicle
  • Completion of a satisfactory Police Vulnerable Sector Check within the last six months
