About the Role

We are seeking a highly skilled and compassionate Social Worker to join our team at Rehab First Inc.

Key Responsibilities
  • Conduct thorough psycho-social assessments to identify client needs and develop effective treatment plans
  • Assign and manage case loads, ensuring timely and high-quality service delivery
  • Develop and implement OCF-18 plans in accordance with Ontario's Statutory Accident Benefit regulations
  • Provide counseling services in various settings, including in-person, phone, and online sessions
  • Maintain accurate and detailed documentation and report writing
  • Collaborate with multidisciplinary teams to ensure comprehensive client care
  • Opportunity to work with adults and children, providing individual, couple, and family counseling
  • Conduct career and vocational assessments and provide guidance
Requirements
  • Master's degree in Social Work (MSW)
  • Current registration with the Ontario College of Social Workers and Social Service Workers
  • Experience in a healthcare setting, preferably in the community
  • Familiarity with relevant legislation, including the Health Care Consent Act and Mental Health Act
  • Excellent communication, problem-solving, time management, and documentation skills
  • Self-motivated and able to manage a busy and varied caseload
  • Valid driver's license, auto insurance, and access to a reliable vehicle
  • Completion of a satisfactory Police Vulnerable Sector Check within the last six months
