Rehab First Inc. is seeking a highly skilled and compassionate Social Worker to join our team. As a Social Worker, you will play a vital role in providing psycho-social assessments, case management, and counseling services to our clients.
Responsibilities:- Conduct thorough psycho-social assessments to identify client needs and develop effective treatment plans.
- Assign and manage case loads, ensuring timely and efficient delivery of services.
- Develop and implement OCF-18 plans under Ontario's Statutory Accident Benefit regulations.
- Provide counseling services in various settings, including in-person, telephone, and cyber counseling.
- Document and report client progress, maintaining accurate and detailed records.
- Collaborate with multidisciplinary teams to ensure comprehensive care and support.
- Master's degree in Social Work (MSW) from an accredited institution.
- Current registration in good standing with the Ontario College of Social Workers and Social Service Workers.
- Experience working in a healthcare setting, preferably in a community-based environment.
- Familiarity with relevant legislation, including the Health Care Consent Act and Mental Health Act.
- Excellent communication, problem-solving, and time management skills.
- Self-motivated and able to manage a busy and varied caseload.
- Valid Driver's License and access to a reliable vehicle.
- Completion of a satisfactory Police Vulnerable Sector Check within the last six months.
