Nurse Care Manager

Role Overview:
We are seeking a dynamic and passionate Nurse Care Manager who wants to be part of the transformational change in home and community care. You will provide comprehensive care in an interdisciplinary team and lead care planning process, working with other disciplines to complete and implement client care plans.
Responsibilities:
- Primary contact for clients when designated as the primary care provider/clinician
- Provision of continuous and comprehensive support to the team in the delivery of care
- Complete client assessments and develop interdisciplinary care plans based on best evidence/practices and client needs
- Facilitate regular interdisciplinary team communication and lead discharge and transition care planning discussions
- Support clients, families, and caregivers through care transitions by collaborating and connecting them with community support services
- Develop strong self-care strategies for clients, families, and caregivers to anticipate and address health issues and optimize their health through effective health promotion strategies
Requirements:
- Registered in good standing with the College of Nurses of Ontario
- Bachelor's Degree in nursing preferred
- Certification in community nursing is an asset
- Minimum of two years of nursing experience, preferably in home/community care, acute care, chronic disease management, health promotion, palliative care, or mental health
- Demonstrated clinical leadership experience and enthusiasm for community rehabilitative care
- Excellent assessment skills, organizational skills, and ability to work independently and in a team environment

Compensation: The estimated annual salary for this role is $85,000 - $105,000 CAD, depending on experience and qualifications.
