About the Role

As a Clinical Team Lead Nurse with VON Canada, you will play a key role in delivering high-quality client-centered health care services. Reporting to the Directing Registered Nurse, this dynamic position involves supervising and coordinating care delivery for a transdisciplinary team in compliance with VON policies and procedures.

About You

We are seeking an experienced Registered Practical Nurse (RPN) who has a minimum of 5 years' experience in community-based health care. As a successful candidate, you must possess strong observation and assessment skills, as well as excellent verbal and written communication skills. Proficiency in Microsoft Office Suite and Windows OS is also required.

Duties and Responsibilities
  1. Supervises the work of the team according to planning guidelines and geographical boundaries.
  2. Works with team members and Management to ensure adequate staffing requirements are met.
  3. Holds regular team meetings and huddles to keep team members appraised of Key Performance Indicators (KPIs) and information necessary for excellent service delivery and team effectiveness.
  4. Provides complete, relevant assessments of the client/family and their environment utilizing established procedures.
  5. In collaboration with the service delivery team, contributes to the delivery of the client's care plan.
  6. Documents problems identified using a specified framework.
  7. Selects and implements strategies or interventions from established care plans in consultation with client and/or caregiver and health care delivery team.
  8. Describes, evaluates, and documents client's responses to care utilizing established procedures.
  9. Utilizes best practice guidelines at all times.
  10. Utilizes adult learning principles and established training materials to teach and enhance skill levels of service providers including Client Care Coordinator as appropriate.
  11. Provides hands-on personal care as required.
  12. Completes home visits with service providers for quality service and competency assessments and to ensure performance at optimal levels to meet client service requirements.
  13. Receives and documents all employee, family, and client complaints/issues related to service delivery and takes appropriate steps to ensure satisfactory resolution.
  14. Meets with individual team members to build education and learning plans and provides input to annual PDP.
  15. Acts as clinical resource to the service delivery team.
