Compassionate Support Specialist

We are seeking a Compassionate Support Specialist to join our team in Ontario. This role offers a unique opportunity to make a meaningful difference in the lives of others while working in a dynamic and supportive environment.
Key Responsibilities:
- Client Support: Provide personal support and homemaking services to clients, promoting independence and overall well-being.
- Collaboration: Work collaboratively with healthcare teams to develop and implement personalized care plans.
- Communication: Effectively communicate with clients, families, and healthcare professionals to ensure seamless care coordination.
- Assessment: Conduct thorough assessments of clients' needs, identifying areas for improvement and developing strategies for enhanced support.
- Documentation: Maintain accurate and up-to-date records of client interactions, care plans, and progress.
Requirements:
- Education: High school diploma or equivalent required; secondary school education preferred.
- Certifications: Certificate in Home Support Worker (Level I, II, III), Personal Support Worker program, or Health Care Aide required.
- Experience: Some household management experience and experience working with elderly or ill individuals or children.
- Skills: Strong communication skills, both verbal and written; ability to work effectively in a multidisciplinary team; good assessment skills; awareness of community resources.
Compensation:
The estimated salary for this position is $25-$30 per hour, depending on experience and qualifications.
