Job Overview
VON Canada is seeking a compassionate and detail-oriented Home Care Support Specialist to join our team in Ontario. This is a permanent, full-time position (1.0 FTE) with a competitive salary range of $43,000 - $55,000 per year, depending on experience.
About the Role
The Home Care Support Specialist provides personalized support and homemaking services to clients in their homes, promoting independence and well-being within the framework of VON policies and procedures.
Key Responsibilities
- Assess and evaluate client needs, developing and implementing personalized care plans to ensure optimal outcomes.
- Deliver personal care and homemaking services, including light housekeeping, meal preparation, and laundry.
- Collaborate with healthcare teams to coordinate client care, ensuring seamless transitions and continuity of service.
- Communicate effectively with clients, families, and healthcare professionals, respecting confidentiality and adhering to strict confidentiality standards.
- Work independently, prioritizing tasks and managing time efficiently to meet client needs and deadlines.
Requirements
- High school diploma or equivalent required; secondary school education preferred.
- Certificate in Home Support Worker (Level I, II, III), Personal Support Worker program, or Health Care Aide required.
- Standard First Aid Certificate or CPR level C Certificate required, if applicable.
- Some household management experience and experience working with elderly or ill individuals or children an asset.
Working Conditions
This role requires a high degree of physical activity, including lifting, carrying, bending, reaching, kneeling, and other movements that emphasize good body mechanics. Individuals in this role are expected to walk, sit, stand, and climb stairs throughout the day, with some tasks requiring fine hand movements.
