Vancouver Coastal Health (VCH) is seeking a registered nurse or psychiatric nurse to join the Intensive Care Management team as a Mental Health and Addictions Outreach Nurse in Sechelt, BC.
This role involves working with individuals with mental health and addictions illness, providing counseling, and assisting clients with reducing repeated contact with healthcare services.
The successful candidate will have experience working with individuals with mental health and addictions illness, a diploma in nursing or psychiatric nursing, and a valid BC driver's license.
The estimated salary for this position is CAD $51,000 per year, based on the location and industry standards.
About the Role- Conduct mental and physical status assessments to determine priority of care requirements.
- Provide counseling and assist clients with reducing repeated contact with healthcare services.
- Assess high risk situations and provide crisis intervention.
- Collaborate with interdisciplinary teams to develop, implement, and adjust client care plans.
The ideal candidate will have broad knowledge of mental health illness and treatment, as well as basic computer literacy to operate a client care information system.

Required Skills and Qualifications- Diploma in Nursing or Psychiatric Nursing.
- Two years' recent related experience working with individuals with mental health and addictions illness.
- Valid BC Driver's License.
- Broad knowledge of mental health illness and treatment.
- Broad knowledge of other facilities and community resources.
- Demonstrated ability to provide treatment planning, counseling, crisis intervention, and case coordination.
