Care Coordinator for Mental Health and Addictions

Are you passionate about delivering exceptional care to individuals struggling with mental health and addictions? We are seeking a highly skilled Care Coordinator to join our team at Vancouver Coastal Health in Sechelt, BC.
About the RoleThis is an exciting opportunity to work as a Mental Health and Addictions Outreach Nurse in our Intensive Care Management team. As a Care Coordinator, you will be responsible for conducting assessments, developing care plans, and providing counseling services to clients. You will also collaborate with interdisciplinary teams to ensure seamless care delivery.
Responsibilities- Conduct comprehensive assessments of clients' mental and physical health needs, identifying priority areas for care and development of individualized care plans.
- Provide one-on-one counseling and support services to clients, empowering them to manage their mental health and reduce reliance on acute healthcare services.
- Develop and implement crisis intervention strategies to address high-risk situations and promote client safety.
- Foster strong relationships with community agencies, healthcare providers, and other stakeholders to facilitate coordination of care and service delivery.
- Maintain accurate records of client interactions, progress, and outcomes, ensuring data-driven decision-making and quality improvement initiatives.
- Diploma in Nursing or Psychiatric Nursing from a recognized institution.
- Minimum two years of experience working with individuals with mental health and addiction issues, or equivalent combination of education and experience.
- Valid BC Driver's License and reliable transportation for local travel.
- Broad knowledge of mental health disorders, treatment options, and recovery strategies.
- Excellent communication and interpersonal skills, with ability to build rapport with diverse client populations and colleagues.

What We Offer- Competitive salary range: $56,000 - $73,000 per annum, depending on experience.
