Job Summary

The Hamilton Health Sciences is seeking a highly skilled and experienced Nurse Practitioner to join our Restorative Care team. As a Nurse Practitioner, you will work autonomously and in collaboration with the interdisciplinary team to provide high-quality patient care. Your expertise will be essential in helping patients reach their maximum level of functioning, independence, and comfort.

Key Responsibilities

  • Provide clinical care grounded in evidence-based standards for hospitalized patients in the Restorative Care Unit
  • Work collaboratively with the interdisciplinary team to develop and implement patient care plans
  • Conduct assessments, diagnose, and treat patients with complex medical and/or functional conditions
  • Develop and implement education programs for patients, families, and healthcare staff
  • Participate in research and quality improvement initiatives to enhance patient care and outcomes

Requirements

  • Current Certification of Registration, in good standing with the College of Nurses of Ontario as a Registered Nurse, Extended Class with Nurse Practitioner (Adult)
  • Master's Degree in Nursing and/or equivalent graduate degree
  • Minimum of five (5) years recent clinical experience supporting a population of acute medical/surgical/complex care/gerontology background
  • Demonstrated experience in research and program development
  • Demonstrated excellent interpersonal and communication skills, and competency to work within interprofessional teams
  • Demonstrated leadership and critical thinking skills
  • Demonstrated ability to promote professional development in nursing
  • Eligible for Faculty appointment with McMaster University

Accountability

Accountable to: Program Director of Complex Care and Orthopedics program and Interprofessional Practice Chief

Works conjointly with the Chief of Complex Care, Aging and Palliative Care and the Complex Care Interprofessional team in the development, implementation, and yearly evaluation of a collaborative practice agreement and medical directives.
