About the Role

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at DTOC III Long Term Care LP. As a key member of our healthcare team, you will be responsible for providing high-quality, patient-centered care to our residents.

Key Responsibilities
  • Primary Care Provision: Collaborate with the Medical Director and Attending Physicians to provide comprehensive primary care to residents, including health assessments, medication management, and diagnostic investigations.
  • Interdisciplinary Collaboration: Work closely with the Director of Care, nursing staff, and interdisciplinary team members to develop, implement, and evaluate residents' plans of care.
  • Leadership and Mentorship: Provide leadership and mentorship to Nurse Managers and clinical staff, enhancing their knowledge, assessment skills, and ability to care for residents.
  • Research and Education: Participate in research, education, and evidence-based practice initiatives to optimize resident, LTCH, and health system outcomes.
Qualifications
  • Education and Certification: Registered Nurse Practitioner in good standing with the College of Nurses of Ontario, or a Registered Nurse Practitioner registered with a regulatory authority in a Canadian jurisdiction, other than Ontario, and holds in that jurisdiction the equivalent of a certificate of registration authorizing independent practice in Ontario as of right exemption Bill 60.
  • Experience and Skills: Comprehensive knowledge of evidence-based clinical practices, long-term care concepts, and philosophies. Excellent interpersonal and communication skills, with experience working as a Nurse Practitioner and in long-term care.
  • Leadership and Supervision: Experience providing leadership and supervision in a healthcare environment, with a focus on promoting individuals' rights and reducing elements of risk.
