We are seeking a highly skilled and compassionate Dementia Support Specialist to join our team in providing exceptional support services to individuals and families impacted by dementia.
Salary: $62,400 - $78,800 per year
This role offers a competitive salary range, reflecting the importance of this position in delivering high-quality support services.
Job Description
As a Dementia Support Specialist, you will be responsible for:
- Delivering Support Programs: Develop and implement comprehensive support programs for individuals with dementia and their caregivers, focusing on education, empowerment, and connection.
- Referral Coordination: Ensure timely referrals to community resources, healthcare providers, and other relevant services, fostering a seamless support network.
- Caregiver Support: Provide one-to-one telephone support, guidance, and resources to caregivers, promoting resilience and well-being.
- Community Engagement: Build and maintain relationships with healthcare professionals, allied health professionals, and other stakeholders to expand referral networks and enhance service delivery.
Required Skills and Qualifications
To succeed in this role, you will require:
- Post-Secondary Education: A degree in a related field, such as social work, psychology, or nursing.
- Experience in Support Services: 3-5 years of experience in delivering support services, particularly in dementia care.
- Strong Communication Skills: Excellent verbal and written communication skills, with the ability to connect with diverse individuals and groups.
- Organizational Abilities: Strong organizational and time management skills, with flexibility to adapt to changing priorities.
