We are seeking a highly skilled Registered Nurse to join our team at Fraser Health, working in our Older Adult Specialized Seniors Clinic. As a key member of our interprofessional care team, you will provide person-centered care to patients, assessing, planning, providing, and evaluating nursing care to promote optimal health outcomes.
Key Responsibilities- Assess patients referred to our clinic, collecting information from various sources and making nursing diagnoses to determine the best course of care.
- Develop and implement individualized care plans, collaborating with patients, families, and caregivers to ensure comprehensive care.
- Provide education and support to patients and families on geriatric health issues, care plans, and illness prevention.
- Collaborate with the interprofessional care team to ensure seamless care delivery and coordination.
- Document patient assessments, nursing diagnoses, interventions, and outcomes according to practice standards and Fraser Health procedures.
- Graduation from an approved school of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives.
- Two years of recent related acute care experience working with a geriatric population, or an equivalent combination of education, training, and experience.
- Ability to communicate effectively with patients, families, caregivers, and healthcare professionals.
- Ability to promote person-centered care, empathy, and patient advocacy.
