Job Summary

We are seeking a highly skilled Registered Nurse to join our team at Fraser Health. As a key member of our interprofessional care team, you will provide direct patient care to older adults in a specialized seniors clinic setting.

Key Responsibilities
  • Assess patients referred to the clinic and collect information from various sources to inform nursing care.
  • Make nursing diagnoses and develop care plans in collaboration with patients and their families/caregivers.
  • Provide education and support to patients and families/caregivers on geriatric health issues, care plans, and health maintenance.
  • Collaborate with the interprofessional care team to ensure comprehensive care planning and delivery.
  • Document assessments, nursing diagnoses, interventions, and patient outcomes according to practice standards and Fraser Health procedures.
Requirements
  • Graduation from an approved school of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Two years of recent related acute care experience working with a geriatric population, or an equivalent combination of education, training, and experience.
  • Ability to communicate effectively with patients, families/caregivers, and the interprofessional care team.
  • Ability to promote person-centered care and integrate and evaluate data from multiple sources to problem-solve effectively.
